Teammate was attempting to open a door at the Blue Creek Loadout. When the employee pulled on the door with EE's left hand EE felt a pop in EE's left arm. During treatment the employee was diagnosed with a full tear of EE's left bicep.

Employee was turning drive sheave to set the thrust on heavy media pump when EE's glove got caught between the sheave and the drive belts resulting in a fracture of EE's right ring finger and 11 sutures and 3 sutures on EE's left pinky finger.

Employee was in the process of putting tools in a toolbox they grabbed a pair of chain hoist off the track ride and begin to walk toward the tool box when they stepped on some draw rock and twisted their left ankle.

EE pulled a bottom roller out from under the belt and stood it up and placed it on EE's shoulder and tried to stand up and carry the bottom roller. When EE stood up EE felt a sharp pain in EE's lower back and left arm and leg.

Employee stated was removing a DPM Filter from the top storage rack. Employee attempted to pick Filter up, left thumb caught on a broken sign causing the Filter to turn over on thumb creating a laceration to left thumb.

Employee stated that they were in the process of loading two bags of rock dust on the scoop on the Unit 1 Section. In the process of the employee lifting up two "40 lbs" bags of rock dust onto the scoop they felt a pain in lower back and left leg.

Employee stated they were in the process of assisting the crew to prepare for a belt move on the Unit 1 Section. In the process of the employee lifting up a piece of top structure EE felt a pain in arm. EE was lifting 48" belt structure when this occurred. NOTE: This incident didn't become reportable until 10/30/2019. This is due to EE going off for surgery.

Employee stated was working on the company #311 10S/C Shuttle Car. Employee was in the process of installing a tie rod end on the dump end of the operator side of the machine on the Unit 2 section. In the process of lining up the tie rod end the employee felt right shoulder pop.

Employee stated that they were installing a new tail piece directly under #2 Mains Head. The crew had accomplished that task and was in the process of pulling the belt back together. In the process of pulling the belt back together a bolt head broke off of a roof bolt striking the employee in the top of the head. The employee received 4 stitches.

Employee stated that Ee was attempting to grease the Joy 14/15 C/M head jack pin on the offside of the C/M but could not reach it from the ground. When Ee attempted to climb up on the C/M the scrubber hood fell smashing hand.

Employee stated ee and another employee were attempting to load a mass jack into a scoop on the Unit 2 section. In the process of lifting the mass jack into the scoop bucket, the jack slipped causing the employee to smash finger between the jack and the scoop bucket. The employee sought treatment and received 10 stitches.

Employee stated they were picking up a 48 inch piece of belt structure on the surface. In the process of picking up the piece of structure they lost their footing in the mud, slipped, and felt a pain in the groin area.

Employee stated ee smashed the tip of right index finger when ee attempted to toss a crib block. EE index finger was smashed between a crib block and side of a scoop. Employee received stitches and modified duty starting 11/14/2017.

Injured employee stated that employee was shoveling #4 mains belt when employee felt a sharp pain in lower back. This is a re-aggravation of a 9/5/2017 incident where the employee was moving miner cable and initially strained lower back. Employee was taken off work on 10/17/2017

Employee stated their right eye turned red and they found a small particle in the eye. Employee tried to wash it out but could not dislodge the material. Employee could not pinpoint work activity that caused the debris to irritate the eye but believed it could have been around the time they were working on some screens.

Employee stated employee had parked the scoop in the #5 entry to dismount so employee could rock dust the #5 heading. The employee got off the scoop and lifted up a bag of rock dust. In the process of lifting the rock dust employee twisted and felt a pain in lower back.

Employee stated with another employee was in the process of dropping miner cable. As they unhooked the cable a rope hanger unwrapped around the cable and hooked the injured employee in the left nostril causing a laceration to nose.

Employee stated EE was dropping bolter cable, the cable had tension which caused the cable to drop fast and caused hand to come in contact with a monkey face causing a laceration to EE's right hand. Employee received stitches.

Employee stated EE was using a cable spool hand crank to clean on magnet. The cable had built up tension. The employee hit the release with a miner bit and the handle came around and struck employees right forearm causing an avulsion. Employee needed stitches to close wound.

Employee stated was pulling a fly pad off of a board to move up. When employee pulled on the fly pad there were loose rocks laying on top of the board. A rock 13"X9"X1.5" thick fell striking employee on the right shoulder.

Employee was plastering a return stopping and was located on the return side of the stopping. While filling a hole with plaster some of the plaster splashed and was blown into eye causing irritation.(Employee was prescribed Antibiotic eye drops)
