The EE was lifting the miner cable and water in the 9 North Mains Section. When EE pushed it up into the roller hanger, EE felt a pop in EE's right shoulder. The EE worked one day, before EE sought out medical treatment.

The employee was moving rail over to the rib of 64 block x-cut in 1WTG construction area. When the rail was rolled, it bounced back pinching the injured's left middle finger between the loose rail and existing track rail. The injury required 2 stitches.

Employee was loosening a crossbar (axle retainer) to replace a chevron on a diesel mantrip. The Bar moved unexpectedly and caught right finger between the bar and metal grading, causing a laceration.

Employee was picking up a can of 320 oil off of a supply car and carrying it two blocks away to the power car. During EE's walk EE felt pain in EE's lower back.

The employee was putting away a jack in a compartment in the motor. The employee raised the lid and must not have locked out the hinge jack, the lid fell and caught the EE's left wrist and hand between the deck lid and sander.

The employee was putting in belt structure on the 1WHG Belt at 101 block. The injured and another employee lifted a bed rail to set it in place. When they went to put it down, the one side shifted and smashed the injured's finger between the bed rail and the feeder. Resulting in a laceration.

EE was working in the 1WHG at 3 1/2 blk building stoppings. EE was using an empty oil can to get water. The water pressure on the outlet forced the lid off and the lid struck the EE's right hand and chest.

EE was pulling face chain on 7 East tear-down. EE was standing back towards the rear of the chain tub, when the chain connecting the face chain to the pull rope broke. A piece of the chain struck the EE below the lip causing a laceration. EE received four stitches and a chipped tooth.

Employee was bending over to retrieve a Big Jack out of the tool tub on the diesel man trip. While lifting the Big Jack out of the tool tub employee felt pain in neck and middle and lower back. Employee was wearing the proper PPE.

The injured was working on 9 North Mains Section 1-2 crosscut at 28 block. The employee bent over to move the miner cable out of the way, when they felt a sharp pain in lower back. Did not violate any laws or company polices, and was wearing all appropriate PPE.

Employee finished hanging ventilation tubing and while stepping down off the miner the employee struck right forearm against an already made up rib bolt with rib plate causing a laceration. The employee received 6 sutures. Employee was wearing all PPE.

Miner was rocking a muck bucket to release tension on the bucket retention bracket. During the rocking process a rock rolled in the bucket pinching EE's Left Index & Middle Finger against the bucket rim. This incident resulted in a fracture to the Left Middle Finger.

The employee was working on an air pump between 8-9 seal of the Main West Seals. When the employee attempted to move a large rock to free an airline EE felt pain in right shoulder. The employee did not violate and laws or company polices. EE was wearing the appropriate PPE and was properly trained.

IE was helping to pump cribs and stepped in void that was filled with concrete. The area hadn't set up yet. IE stepped into void area sinking to knees causing IE to get stuck. Other employees helped IE out.

The employee was dragging ventilation tubes towards the miner, when the tube caught on the tire of the FCT-(Flexible Conveyor train) jerking the employee backwards causing a strain to lower back. The employee did not violate any laws or company policies and was properly trained.

The employee was working on 6 East L/W Section delugging track ties. The employee had the rail jacked up, and placed a header board sideways to try and support the rail. When the employee went to remove the track tie, the header board kicked out and the rail came down the on employee's right foot. The employee did not violate any laws or company polices.

The employee was bolting on 6 East Belt line. Drilled a hole for a 16' cable bolt, after removing the steels noticed EE's left shoulder was sore. The employee did not violate any laws and was wearing all the appropriate PPE.

The employee was cribbing toes of shields on the L/W face. As the employee was putting cribs under the toes they smashed their fingers between the toe of the shield and the crib. The employee received eight stitches.

A scoop that was operating approximately 350 feet away struck the track causing the track to shift and a cross tie then struck the employee causing a laceration to the small toe on the right foot. The employee received three sutures to close the wound.

The employee was breaking down a Pump, and changing it out with an air pump. At the end of this process the employee had pain in lower back. The employee was wearing all the appropriate PPE at the time of the incident.

Carrying 20' joint of 6" waterline down 6E belt line outby the recent belt move (6E #3 entry) when they tripped and fell, dropping the pipe on their neck resulting in a strain/sprain type injury.

Injured was drilling roof in 6E section 2-3 crosscut, when they took the steels out and turned to place them on the bolter tray they bumped right elbow on the bolter frame. Reported the accident the next day, later diagnosed with a staphylococcus infection in the elbow, that may be related to the injury.

The employee was laying rail on St. Leo Section. The employee was holding a piece of rail with a slat bar, when another employee moved the rail over against EE's foot causing EE's knee to buckle. The employee worked until EE had surgery on 3/11/19. The employee did not violate any laws or company policies and was properly trained.

Injured got out of loader (9 North mains #3 entry) to move cable out of the way while the miner was tramming inby at the face. Miner cable splice got caught in roller hanger, tightening up and causing hanger to fail causing cable to snap down onto injured employee's hard hat, resulting in neck pain.

Bolting off right side of miner in 5E in the 4 left entry and was pushing roof bolt into the drill hole when the bolt coupler struck the roof strap causing a sudden stop which caused the miner to feel pain in the right shoulder. Diagnosed with unspecified sprain of right shoulder joint.

After working dayshift, the injured employee was in the hourly bath house at the Miracle Run Portal. When EE went to lower the clothing basket, a pick hammer dislodged and fell from a hanging basket and struck EE on the top of the head and causing a laceration that required one staple to close.

While picking up a bundle of bolts to transfer from a scoop to a shuttle car the injured felt pain in lower back. Injured has a history of previous lower back issues from a previous injury at a different mine. Incident occurred on 5 East section 3-2 in last open crosscut.

While driving shuttle car in St Leo #2 entry 5 block of St Leo North Mains ee discovered a bolter cable had fallen out of a "Christmas tree" hanger and was laying in the haul road. After lifting the approx. 10' section of cable into the hanger (approx. 7.5' off mine floor) ee felt pain in the left side of chest. Diagnosed with strained 9th rib.

Employee was helping crew members remove plywood after making a vulcanized splice in belt, area was low, employee struck hardhat on an exposed bolt shank causing injury. Cause of this injury was paying attention to task at hand. Compliance with rules/regs, mining equip/systems not a factor, Proper PPE (hard hat) may be a factor along with miner proficiency.

Unloading rollers for 11 belt from the passenger compartment of a diesel mantrip parked on 9 south haulage near 11 drive and felt pain near left scapula and right rotator. Diagnosed with strain to soft tissue of left scapula and right shoulder.

Allegedly laying under 14 belt pulling belt over bottom rollers to make splice and felt pain in chest and back. Originally turned in under comp for 2/15/18 incident report where EE fell down. Incident report for this event not completed until 3/1/18. Alleged incident is still under review.

Injured was retrieving ventilation tubes from #2 entry of 5E (inby last open), standing with tubes on left shoulder, went up on tip toes to unhook hold up chain and felt pain in abdomen, allegedly resulting in an abdominal strain that irritated 2 pre-existing (non-work related) hernia's. No witnesses. Incident is still under review.
