Injured employee was supervising the installation of a longwall tailgate winder on the 2E set-up face. The winder was positioned & EE was inspecting the bottom mounting plate when it unexpectedly shifted & rolled onto EE's right index finger. The tip of the finger was fractured and cut requiring 3 sutures. Injured didn't violate any rules or regulations & was wearing required PPE.

Employee was shoveling spillage at a feeder in the Prep. Plant. As EE was shoveling coal fines onto the belt EE twisted and felt a "pop" in the left side of EE's abdomen. EE later discovered a knot had appeared in the area. EE didn't violate any rules or regulations & was wearing required PPE but failed to use proper shoveling technique. Employee has been diagnosed with a hernia.

Injured employee was carrying a joint of 6" pipe through water in front of a personnel door. As the injured employee was pushing the end of the pipe through the door, they slipped and fell against the door frame and the air pressure against the door caused it to slam against employee's head, neck and left shoulder.

IE was assisting with the installation of a roof channel on a continuous miner. EE advanced channel toward the head as the CM operator was retracting the CM head. As the head came back it hit the inby end of the channel causing the outby end to kick up against the roof. Injured was holding the outby end when the strap hit the roof & the distal part of EE's thumb was amputated.

Injured employee was roof bolting on the right side of the continuous miner. As the miner was advancing a vent tube fell out of the holder and employee tried to catch it and strained their back. This employee began missing work on 03/02/2020 due to this injury. Employee didn't violate any rules or regulations and was wearing required PPE.

Injured employee was removing belt cradles from a scoop bucket. Five cradles were banded together with plastic bands. Employee began cutting the bands while holding the bundle with left hand. When the bands were cut, the bundle started to fall over. Employee tried to hold them up with left arm but the cradles were too heavy and pulled employee's left arm.

EE was loading roof bolting supplies from a scoop bucket onto the continuous miner when EE felt a pain in right shoulder. EE was examined and was told EE pinched a nerve/strained a muscle. EE retuned to work next shift. EE didn't violate any rules or regulations and was wearing required PPE.

EE was attempting to remove the hose from the walkway when EE felt pain down the neck, shoulder and right arm. Injured did not violate any rules or regulations and has proper training. Was wearing required PPE.

While walking down #3 entry of 1 West Tailgate (carrying airline past pumpable cribs), near 75 block, the injured slipped on pumpable crib slurry that was spilled on the mine floor, causing them to injure their left ankle.

Injured employee was hanging the continuous miner cable up in a hanger from the mine roof in the #1 entry of the 2-W CM Section. As they were hanging the cable, they lost balance and stepped backward into a rut in the haul road. Employee sprained the meniscus in right knee. Employee didn't violate any rules or regulations and has proper training. Was wearing required PPE.

Injured employee was assisting in changing out a leaking 8" waterline coupler on the 4 West Belt at 34 3/4 block. While reaching/lifting overhead, EE felt pain along the right side of neck and right shoulder/arm. The employee did not violate any rules or regulations and has proper training. Was wearing required PPE. Employee began missing work on 7-24-19 due to this injury.

Injured EE was installing the pin in a relay bar on the 1-W Longwall set-up face. As EE was aligning the pin for insertion the relay bar unexpectedly shifted upward pinching EE's right hand between the pin and panline, causing a laceration to EE's thumb requiring 9 sutures. EE was wearing required PPE and did not violate any regulations.

Injured employee was entering the Prep. Plant Control Room. Opened door to room & stuck EE's right hand back to catch the door as it was closing behind EE. EE inadvertently stuck EE's right little finger in the hinged side of the door as it closed, amputating the end of EE's little finger just behind the fingernail. No regulations were violated.

The injured employee was helping lift a longwall shearer ranging arm pin when ee felt a sharp pain in back. Has pain and stiffness in neck and back. Injured employee did not violate any rules or regulations and was wearing required PPE.

The EE was lifting a tool bag off a shelf in the elevator. As EE lifted the bag it dropped pulling EE's right arm down, causing pain. The accident was caused by the EE failing to maintain control of the bag while lifting it. EE began missing work on 3/11/19 due to this injury. The EE was wearing required PPE, has training and experience. No rules or regulations were violated.

EE was walking down the #3 entry carrying 2, 27" pumpable can crib bags. As EE was walking, they stepped on a loose piece of coal/rock and rolled their left ankle, causing them to twist & fall to the mine floor. EE suffered a left ankle fracture & dislocation as a result. EE was wearing required PPE and didn't violate any rules or regulations.

They twisted as they were lifting the waterline up onto a high area on the 4-E longwall belt conveyor. Employee didn't violate any regulations and has experience in the task. Employee was wearing required PPE.

Injured employee was loading belt roller cradles onto a mantrip for transport. As EE was loading a cradle EE inadvertently smashed EE's fingers between the cradle and one EE had previously loaded. Employee was unaware of EE's hand placement between the 2 cradles. EE didn't violate any regulations and was wearing PPE.

Injured employee was roof bolting on the 1W Tailgate Section. As ee was installing an 8 foot roof bolt, using left hand to push the bolt up into the bolt hole ee felt pain in left shoulder. EE continued to work until 10-5-18 when ee began missing work due to this injury. Employee has training & was wearing proper PPE. Did not violate any rules or regulations.

Employee has continued to work until 10/25/18 when employee began missing due to having surgery on injured shoulder.

Employee was carrying a top belt roller up a ladder to replace a bad top roller, when they felt a "pop" in the lower back. Complained of tightness and tingling in the lower back. The employee continued to work until 8/13/18 when they began missing work due to this injury. The employee did not violate any rules or regulations and has proper training and experience.

EE was dragging belt roller structure to locations along the belt entry for replacement when EE began feeling sharp pain in lower back. Injury caused by EE failure to use proper body mechanics and overexerted the back. EE did not violate any laws and was wearing proper PPE. Has performed task before and is properly trained.

Employee was stacking cinder blocks and smashed EE's left small finger between 2 blocks causing a laceration which required 8 stitches. Employee has experience in job and was wearing required PPE including metacarpal gloves. Employee did not violate any rules or regulations. No equipment was involved in this accident.

No mining equipment was involved and employee has proper training. Received information on 4/2/18 that employee has been diagnosed with a hernia. Employee has not missed any work due to this accident at this time.

Injured EE was carrying a belt roller on EE's right shoulder in the Prep. Plant. While walking around a corner, the end of the roller hit the corner and fell off EE's shoulder causing EE's right hand to bend and twist backward. EE continued to work up to 1/25/18, where EE had a x-ray and it was determined EE had a fracture in EE's hand. The employee then began missing work.

While the EE was changing the crawler mobile tailpiece valve bank, the individual got solsenic in eyes, nose, and mouth. The pumps were shut off and pressure was bled off of the system. The employee was wearing all required PPE at the time of the incident. The EE was properly trained in the task being performed.

No mining equipment was involved and employee has proper training and was wearing required PPE. Received information on 11/13/17 that employee has been diagnosed with a hernia. Employee has not missed any work due to injury at this time.

Employee was lifting a bag of garbage up into a dumpster and strained their left shoulder. Employee began missing work on 10-30-17 due to this injury to have surgery to repair the rotator cuff. Employee didn't violate any rules or regulations and has proper training. Employee was wearing appropriate PPE.

Employee did not violate any laws, rules or policies, is properly trained, and wearing proper PPE. Has a good attitude for safety.

Employee has a good attitude for safety. EE did not have a specific incident and is believed this is a result of a body failure type injury. Employee has proper training and was wearing required PPE.

The accident was caused by the employee's failure to pay attention to the area EE was walking in. Employee was wearing all required PPE but needs to improve attitude for safety.

The employee has a good positive attitude and was not violating any rules, regulations or policies. The accident happen when an unexpected leak occurred.

The accident occurred because the EE failed to maintain a firm grip on the cradle. EE was wearing the appropriate PPE. EE is properly trained, has a good safety attitude. EE did not violate any rules, regulations or policies.

The employee stated that right hand slid along a metal roof strap EE was loading onto a scoop. EE received a laceration to right middle finger which required 5 sutures. The accident occurred because the employee slid hand along the sharp edge of the strap. EE had on the appropriate PPE and did not violate any rules regulations or policies. EE is properly trained.

Employee was using a drill press to take bushings out of a steering arm. The part started to fall off press table and struck EE's hand. Cause of this injury was not seeking help to stabilize and lift the heavy arm. Compliance with compliance/rules, PPE, mining equip/systems was not a factor. Miner proficiency may have been a factor.
