On 1/25/2026 EE stated that while EE was bending a 5' bolt EE felt a pain in EE's right shoulder. EE was granted a full release and returned to work. On 3/30/2026 Mine Management was informed that EE started Physical Therapy for EE's shoulder.

EE was dropping CM cable and water line from a hanger when EE felt pain in EE's left shoulder.
Employee was removing hose from axle cooler pump on 993k Z9K00314 when pulling hose back for removal, hand slipped making contact with top of transmission. Unknown exactly what the right hand contact on transmission. The contact caused a laceration on top of right hand
Employee stated that they reached down to move a 5-gallon oil can off the CM cable. While grabbing the can, they cut their right index finger. The laceration required 5 sutures.
EE stated that B-Bond got on EE's pants and caused EE's thighs to get irritant contact dermatitis.
Employee stated that while they were sliding the CM cable over with their right foot, they felt a pain in their right knee.
EE stated that EE had lifted the shield on the #1 battery operated 4-wheeler, the panel slipped out of EE's hand causing the panel to fall on EE's left index finger.
Stepping up onto front-end loader, felt a pop in knee.
EE stated was unhooking a chain from the CM conveyor pan when a burr cut through EE's glove and caused a laceration to the left hand.
Employee stated that they were moving the CM cable closer to the rib when they felt a pop and instant pain in right shoulder.
EE stated that the Motor quit pulling so EE raised the lid on the motor. While EE raised the lid and it slipped out of EE's left hand and fell and struck the index finger on EE's right hand.
Employee was carrying two cans of hydraulic oil when one of the handles came off and they tripped over the can. This caused them to fall and strike right shoulder on the track rail.
Employee was hauling track rails off the # 1 Unit with the motor and rail car when a rail shifted. EE attempted to swing the rail back over by hand when a rail suddenly rolled over and pinched EE's left middle finger between two rails on the rail car.
EE stated that while EE was changing batteries on the scoop, EE grabbed the jumper cable that was connected to the scoop plug and pulled on the cable. The jumper plug suddenly disconnected and hit EE in the mouth thus causing a chip in EE's front right tooth.
Trying to pull door open that was frozen shut. This was at starting time and door had frozen shut during the night.
Loading oxygen bottle onto flat car and lost EE's grip. Tried to catch bottle mashing EE's finger between the bottle and the metal part of the ratchet strap.

Employee was lifting a rock on the side of the shuttle car to place in boom. The rock tipped up and pinched EE's finger against the mine roof resulting in abrasions and lacerations to the left index finger.
Employee was assisting in mine belt recovery; placed a hand on sheave rope and turned their head, at this time the left pinky finger made contact with the sheave pulley which resulted in an amputation of the left pinky finger at the first knuckle of the finger.
Was looking for oil leak around CM pan and accidentally set CM pan down on foot.
