Two workers were attempting to get a coupler off a plastic pipe. One worker's hands slipped as they tried to turn the coupler and their left ring finger hit a pipeline component or the pipe wrench causing two cuts to this finger. Sutures were applied.

Showing all 5Employee was cleaning off a 25' tape measure & trying to get it to roll back up. The tape measure was getting stuck when rolling back up. The tape measure suddenly started to roll up & cut vertically across the top pad of the right thumb. EE cleaned & bandaged their thumb & went back to work. Later they went to the ER received 5 sutures.
Moving a hose clamp without gloves, working alone, and wet conditions existed.
Individual was placing a sump pump near ball mill 4, placed hand in a pinch point. Resulted in smashing left index finger.
Individual was washing a beaker that had a chip in the top. Resulted in a laceration on hand between thumb and index finger.
