Employee stated was working with the power movers hanging miner cable when EE felt pain in the lower back and left leg. Employee came back to work on regular job but started missing on 9/24/20.

Employee stated EE was bolting the #10 entry. The top and bottom pinner steels were stuck together and when EE pulled them apart pulling towards EE's face the pinner steel hit EE in the chin requiring 4 sutures.

Employee stated EE was working on the belt crew putting on belt when EE felt pain in the middle of EE's back. EE was pulling on a piece of belt.

Employee stated that the wet down hose was pushed into the feeder and became wrapped around the pit breaker. The power was locked and tagged out and employee was manually rotating the pit breaker unwinding the hose when right hand got caught between the frame and a pit breaker bit. The employee received sutures in right hand.

Employee stated EE was breaking a header board apart when EE hit a metal band and the band hit EE's right wrist causing a laceration requiring sutures.

Employee was bleeding air out of a 6" waterline at the fire valve on #3 Unit. The waterline had just been installed at the new unit. The employee was opening and shutting the fire outlet valve when the last section came apart at the last joint which was 20' away. The belt was chained to the roof but lunged forward striking the employee in the nose. This resulted in a broken nose.

During third shift belt advancement shift, miner's left hand became caught between belt tailpiece and wooden timber resulting in fracture in left small finger.

Employee who had previously had a hip replacement approximately 6 years prior was building permanent ventilation stopping. Employee had completed stopping and began to seal with plaster. Employee picked up bucket of plaster and felt sharp pain in left hip area.
