While conducting an end-of-month inventory of explosives supplies, Miner unlocked the powder magazine in Tunnel 1. Miner opened the door using their left arm to pull the heavy steel door open. As they opened the door they felt a pop, and a sharp pain in their left forearm. MRI results received on 9/9/2025 showing injury was a tear requiring surgery.

THE IE REPORTED BACK STRAIN ON 8/18/25 AT THE START OF SHIFT. IE STATED THAT THE INCIDENT OCCURRED ON 8/13/25. THE IE WAS REMOVING A BROKEN PALLET FROM THE BAGGING SYSTEM WHEN THEY FELT A STRAIN IN BACK. IE WAS SEEN BY PHYSICIAN AND TAKEN OFF WORK.

While removing tires from a dump truck to perform maintenance, the miner felt a pop in EE's left shoulder and a slight tingling in EE's arm. Injury occurred on 05/12/25 and treated as first aid until 06/20/25. On 06/20/25 it was diagnosed as a tear requiring surgery and miner was given restrictions.

Miner was assisting to shift the width of the forks on an extenda-reach forklift. While moving the forks, the miner felt a pain in EE's right forearm. The injury was initially treated as a first aid sprain, but the extended healing process led to a need for imaging which revealed a tear requiring surgery to repair. The determination that surgery was required was made on 5/16/25.

While bagging a load of 55-lbs. bags and placing them on a pallet, felt an intense sharp pain and since then whenever moving the arm there is a grinding/popping feeling and discomfort. The injury was originally diagnosed as first aid and treated accordingly. On 10/30/2024, the pain had not lessened and the doctor prescribed medication and work restrictions.

IE was lifting a mop bucket to dump in a sink and felt a pop in EE's back. IE refused treatment on 10/21/24. On 10/22/2024 IE sought medical treatment and was given two injections and was taken off work...unable to accommodate restrictions.

The operator sustained a strain injury while installing a new propane canister on a forklift. During the installation the operator did not feel any significant strain or pulling, but immediately after completing the process they began to feel discomfort.

Maintenance member was replacing a belt on a mill fan when their fingers were pinched between the belt and the belt sheave. Their left index and middle finger received lacerations requiring a total of 13 stitches.

The miner was injured on 5/16/24 while moving a chair from on top of a desk. Per doctor, the injury was treated as a first aid strain until 8/5/2024 when imaging revealed a tear and surgery determined necessary.

WHILE OFF ON PERSONNEL MEDICAL HAD MRI ON RIGHT ELBOW, REVEALED TORN R BICEP TENDON TEAR. IE RETURNED FROM PERSONAL MEDICAL, WORKED UNTIL SURGURY. FIRST SCHEDULED WORK DAY MISSED 8/26/2024.

The IE was moving a stack of pallets through the "speed door" in the Packaging Plant when saw dust came off of one of the pallets and into the IE's right eye. The IE was seen the following day by an OD who removed debris from the eye and prescribed antibiotic drops.

Employee was loading a bulk truck. When trying to open a lid one of the hatches was bound up/stuck. The employee could not find a "hatch tool" and tried to manually force the latch open. Employee felt a pain in shoulder but finished the shift. Incident was reported the next morning when the pain had not gotten better.

EE was filling super sacks with product in the packaging plant. As the super sack was going down the conveyor, EE noticed the sack was leaning. EE stopped and tried to push back on the sack and felt a pop in EE's left knee so EE was taken to ER. EE can't get into a Dr. to be released due to COVID-19 policies so EE has missed too many days of work.

Miner was attempting to remove a frayed piece of rubber from a haul truck tire by pulling on it. When they pulled they felt a "pop" in right shoulder followed by minor pain. Our mine Supervisor took the employee to a local med clinic and they receive a steroid shot and some minor restrictions for a sprain.

IE was attempting to hook up a mobile air compressor by unhooking a stationary air compressor so that the stationary air compressor could be serviced. As they started to hook the 3" hose up, they tripped over the excess hose on the ground and fell. As they fell, left thumb hit the framework and caused a fracture to left thumb.

Miner was helping with a derailed rail car. EE reached under railcar to move support. While pulling hand out from under rail car EE's left hand contacted a bur on the metal of the car caused a laceration to the top of EE's hand near the thumb. Miner was not wearing gloves at the time.

While re-installing a guard, the employee let go of guard with one hand to re-position. Before ee could grab the guard again ee dropped it. The guard fell onto right index finger resulting in a fracture.
