IE was helping repair a 4' heavy-duty hose that feeds the dewatering system's #7 shaker. IE was holding the hose when it shifted, striking and pinching IE's right pinky finger. IE was taken to emergency room where it took 4 sutures to close the wound.

Showing all 35IE was helping set feeder in place and had EE's foot underneath load end of feeder. IE's foot was smashed when feeder was set on the ground. IE was taken to emergency room where EE suffered two fractures on EE's left foot.
IE was doing normal work duties and felt pain in both shoulders. IE continued to work full duty up until December 5th and missed their first scheduled shift.

IE was hanging high volt cable and was tightening down strap. IE hand slipped off of strap and high volt nip struck employee in the chin causing a laceration.
Employee was coupling together a hose above EE's head. One end of the hose slipped out of EE's hand striking employee on the front tooth. Employee's tooth was chipped during the incident.
Employee was using a washdown hose and when the hose became pressurized. Employee didn't have control of the hose and hose started whipping around striking employee in the left eye causing bruising.

I.E. sustained amputations to the ring, middle and index fingers of the left hand while in the process of lifting a Longwall Gate Shield tip up. The fingers were caught between the shield canopy and a 6x6 timber that was being used to keep the tip propped up.
Employee sustained injuries to the tip of the right index finger while removing belt structure. The finger became caught between a belt rail and spreader bar causing a fracture and avulsion.
I.E. was picking up old belt structure, slipped and fell injuring both knees.
I.E. was lifting belt frame, felt pain in right hamstring.
Worker was relocating bottom rollers, when one rolled off onto a foot. Worker was given aid and released with a bruised foot
I.E. was installing a bearing cap over a tail roller bearing, the cap slipped and smashed the left ring finger between the cap and housing.
I.E. was loosening a Belt Clamp that was under tension from the belt. The belt started to slip and then grabbed the clamp again which caused it to jerk and the clamp come off of the bars. The clamp came off with force and struck the I.E. in the left upper arm.
Employee twisted their right ankle when carrying a de-water pump to the Longwall face.
Employee was loading bolts from the back of the roof bolter to the tray when sharp pain in the lower back occurred.
I.E. was going through a man door to access a return entry. The I.E. lost footing causing their hands to slip off the door and the door slamming shut. The I.E.'s tip of the left middle finger was caught between the door and door frame.
Employee was pulling in a Longwall shield manually when a rock fell from between the adjacent shield striking the back of the right hand.
Employee was in the process of changing out a dewater pump at the boot wash station at the bottom of the shaft. They accidentally stepped into the pump hole when working in the area and submersed their leg up to the knee.

When done filling the employee pulled the hose from the tank and accidentally opened that valve, which caused the hose to get away from them. When trying to contain the hose to shut of the valve the hose came around and struck the employee in the right ankle.
Employee was installing slider beds on a Richmond load bed. While positioning a bed, it shifted and the employees left ring finger was caught between it and the previously installed bed.
