Employee stated that while pulling belt structure they twisted and felt pain in their right knee. Employee received medical treatment on 3/6/2026 for the injury. Employee has not missed any scheduled shifts as a result of the incident.

Injured employee was attempting to push a rock off the cable tray of the longwall panline when EE caught EE's left index finger between the rock and shield canopy resulting in a fracture to the tip of EE's finger and a laceration requiring sutures to close. No time was missed as a result of this incident.

Injured employee was handling block while building a stopping when EE felt pain in EE's left knee. Employee started physical therapy on 11/24/2025. No scheduled shifts were missed as a result of this incident.

Employee was dragging mine spray with a crew when another employee unexpectedly dropped the end causing a right shoulder strain. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.

Injured employee was attempting to separate drill steels when the steels came apart and struck their upper lip, chipping their front tooth requiring dental repair. No time was missed as a result of this incident.

Employee stated that after picking up a battery backup and a fixed mesh node for the wireless tracking/communication system employee felt a pain in left shoulder. Employee continued to work until 8/7/24 when employee was taken off work for surgery.

Employee was cleaning up spillage on the longwall face and picked up a chunk of coal to throw back into the panline and got finger caught between the panline and chunk of coal causing laceration.

Injured employee bent down to move the continuous miner cable and felt pain in lower back. The injured employee continued to work until 10/16/23 when the employee was taken off work by a physician.

Injured employee was assisting a co-worker with positioning a steel plate on a supply car when the plate shifted and caught their right third finger resulting in a fracture and laceration. No time was missed as a result of this incident.

Injured employee was operating the company #1035 scoop on the #1 Headgate section removing trash from the section. While unloading the trash from the scoop employee felt pain in left knee and was unable to complete the shift.

Employee stated that while unloading a steel beam off a diesel forklift EE caught left ring finger between the beam and the forks resulting in a laceration requiring sutures to close. Protective gloves were being worn by the employee at the time of the incident.

Moving a full sheet of grating across the top of steel to set in place using a separate lanyard when grating fell into an opening. EE hooked the grating and wrapped the lanyard around EE's arm. It fell through an opening, pulling EE to the edge and arm through the floor opening. Upper Right Arm broken w/transverse fracture.

Employees were removing damaged fan line. During the process of hooking up rigging, an employee hooked rigging on loose tail end of cable. Once the cable took weight of the rigging, it pulled the assumed connection point, allowing rigging to fall to work deck hitting employee.
