Injured was helping change conveyor through rollers and cut his left hand on sharp edge of roller as roller slipped from his grasp. Injured was wearing gloves and still cut palm of hand. He needed stitches to close laceration. He lost no time.
EE was opening a metal container of welding rods, when he took his glove off to get a better grip he cut his finger on the edge of the container. EE needed four stitches to close the laceration. Working light duty, working the same hours.
Injured ee met with foreman prior to going to plant to assist in removing shaker guard to replace drive belts injured ee told foreman that morning that his back hurt he though that it was possible his kidneys, but he would be able to work the day. just after removing guard, EE now claims he strained back in same area of initial pain. Employee was exiting Cat 345 Excavator when he slammed door on his right ring finger causing a laceration that needed stitches.
Ijured was working on floor (shop) to repair pulley. Crusher rotor tip that was sitting on table, adjacent to where he was working, fell off table top and struck injured on left hand. Fracture left middle finger.
WHILE TRYING TO REPLACE CABLE ON DREDGE SPUD, INJURED GOT LEFT INDEX FINGER CAUGHT BETWEEN SPUD SLEEVE AND STEEL BAR. INJURED SMASHED TIP OF INDEX FINGER.
ON 12/30/02 INJURED REPORTED FEELING DISCOMFORT AND PAIN WHILE TRYING TO FLIP AN IRON PIPE BELL THAT HE WAS RESURFACING. DOCTOR ORIGINALLY DIAGNOSED A GROIN PULL. NEW DIAGNOSIS SHOWS INGUINIAL HERNIA TO BE REPAIRED 1/16/03.

EE WAS BENT OVER INSTALLING WATER COOLER ON EQUIPMENT. WHEN HE STOOD UP HE FELT PAIN IN LOWER BACK.
