In attempting to remove 150lb guard for install, injured EE hand was in line of fire when other EE lowered guard thinking they were out of the way. Injuries: Fracture right ring finger, lacerations to right ring and middle fingers rq. stitches

The worker was replacing a bad bearing on SR-02A. The worker had to perform lifting and hammering during the removal of the old bearing and installing the new. The front line maintenance worker's left elbow is sore. The new bearing weighs 87.5lb.

Employee was setting up 24-ft extension ladder at the fuel shed, to check gauge on the diesel tank. Employee was working alone, taking ladder from bed of Maint truck to lean against tank, when they felt a pop in right shoulder and pain. Employee states that they have extreme pain when they lift arm over shoulder height. Appointment with Dr was 30 Aug 22.

While working in the Air Separator (GSAS-01) at the Grinding Mill, a maintenance EE's hands slipped of a Whizzer bar EE was removing. There was a sharp piece of metal protruding which lacerated the EE's right thumb between the 2 knuckles. EE received 4 stitches to their thumb.

Employee was on ladder of railcar, reaching for a portable electric hoist (~20-25 lbs), grabbed the hoist and rotated EE's left arm (with the hoist in hand) to hand off to another operator, when EE felt a pop and pain in EE's upper left shoulder. Employee went to the emergency room that day and was placed on light duty, to include no lifting, pushing or pulling.

The lead mechanic/electrician reached out to pull electrical wire to place on a pole and heard right shoulder pop. First visit to a medical doctor was Monday 06/07/2021. During this exam the mechanic was placed on restricted duties.

A Relief Operator while closing a valve in the crushing area felt a pop in left elbow. Employee immediately reported the occurrence to supervisor. As the shift progressed the pain, motion, and hand grip strength worsened causing the employee to request a medical evaluation.

The employee was pushing with right foot (up to 20 lbs. of force) on a metal truck inspection ramp connected to two cables with counterweights and heard a pop and felt pain in right knee. After months of therapy and an MRI, it was determined the employee had a torn meniscus. The employee worked up to scheduled surgery on 09/23/2021.

Employee was attempting to clean out the sump hole at the west end of the reclaim building. When EE tried to lift the sump pump from the hole EE felt a pop in EE's right forearm. Employee reported the incident to EE's supervisor and finished EE's shift. The following Monday (9/14/20) the employee visited a doctor who placed EE on modified duty and suggested an MRI.

Thursday April 30th while installing dryer flights at the rotary dryer (DR-01) the maintenance employee strained back. EE did not want to go to the doctor and finished the shift. Friday May 1st the employee was able to come to work, stretch it out and make it thru the day. Thursday May 28th the employee stated EE needed to go to the doctor because back was injured.

Employee doing maint work on caustic pump in Flot bldg. EE leaned over the pump, a pin hole leaked on the caustic line of pump, spraying caustic soda 50% membrane under employee's face shield. Caustic hit EE's L side lips and EE's R eye. At the hospital, found with no lasting injuries, returned to duties, with no modifications, but w/ prescription eye drops for irritation.

Employee assisting to feed belt onto conveyor at head pulley. Employee bent over guard at head pulley to pull belt when they felt sudden pain in lower back from reaching across. Employee refused Workers Comp claim/appt 4, 11, 12 Feb. and agreed to claim/appt Feb 17. Appt found lumbar strain; not allowed to lift over 25 lbs, bend, stoop, or twist until follow up.

The employee works in a position where frequent shoveling is required. EE's shoulder has been sore for a while and aggravated by shoveling. Is experiencing tingling in EE's arm when trying to sleep. Went to Dr. who says scar tissue has built up and prescribed 6 weeks of physical therapy.

Employee was replacing a troughing roller on BC54 at the head end of the belt. While turning to pick up a bolt to fasten down the new assembly EE hit knee on the torque arm support for the reducer. This caused EE's knee to flex sideways. EE was experiencing some swelling and soreness. Update: Went to doctor on 9-20-19 and scheduled an MRI for 9-25-19.

Contacted on 5/08/2019 by a OTR trucking company that one of their drivers had allegedly been hurt on our site. The driver did not report the injury to anyone at the time of the incident so details are limited.

On 8/6/19, the employee needed left shoulder surgery was deemed as work related by Workers Comp Ins. On 03/27/19, the employee reported pulling/straining EE's left shoulder on 03/25/19 when climbing a ladder seven time to adjust chemical pumps instead of using stairway. No medical attention needed or requested at the time. Date of first day of lost Time 08/19/2019 due to surgery.

Employee had EE's left thumb struck by a spring-loaded gate while descending a ladder in the Plant # 2 processing plant. EE was examined and diagnosed with a contusion and was prescribed an anti-inflammatory. Employee inattention and failing to SLAM contributed to the incident.

Employee was removing a blow bar from the Hazemag secondary crusher on 12/21/18 and a tiny sliver hit their finger. They initially felt no pain however it started to hurt on 12/23/18 when they reported the incident. They had a medical procedure on 12/ 28/2018 that required it to be reportable. They did not miss work.

EE was stacking bags on a pallet in the bag house. As EE was finishing the top row EE felt a pop in the left shoulder and felt pain. EE notified the S&H supervisor about the injury and also informed supervisor that EE had a sports injury to the same shoulder in 2005 while EE was in high school.

Employee was performing housekeeping activities in our warehouse. EE was assisting placing an electric motor on a pallet that already had one motor in place. When setting motor on pallet their thumb and 4th finger on left hand was caught in-between the two motors. No treatment, 4th finger, the thumb did have to be drained from swelling.

The worker was in the process of re positioning a 3 inch pump discharge line on PU-53. The line weighed 10lb or less. During the lifting process EE placed both hands under the pipe and locked EE's fingers in place and EE felt a sharp pain in EE's right bicep. The worker thought it was a cramp but was transported to the emergency room and was diagnosed a torn bicep.

Maintenance EE was replacing a flow divider for the mill hydraulics. EE loosened the hose for the flow divider and hydraulic fluid splashed ee in the face and went under safety glasses and into eyes. EE flushed eyes and was taken to the ER. ER flushed eyes again and prescribed prescription eye drops. EE returned to work with no modified duty or lost time.

While rebuilding a valve a board the valve was resting on moved causing the valve to fall. One operator tried to catch it and the valve pinched EE's finger between it and the rim of a sump resulting in a cut to the middle finger of EE's right hand. EE was treated with seven stitches and released back to work with no restrictions. EE returned to work and finished EE's shift

Ee went to check on the 510 air valve in the tunnel. Ee rested EE's hand on the upper ledge of the tunnel entrance. A small amount of sand fell from the ledge getting into the EE's right eye. Eye was flushed. EE went to the emergency room the next day and received a prescription for eye drops. Ee returned to work. No modified duty.

EE was attempting to remove a fan blade from FA-1301 and was struck in the face with the blade. EE had the fan locked and tagged out. The fan slid off the shaft as EE was removing it and was struck in the face.

Employee was performing quality control sampling and noticed something irritating employee's right eye. Employee was taken to the local clinic to irrigate employee's eye and nothing was found to be in employee's eye. Physician prescribed eye drops as a preventative measure for possible infection.

EE was helping pick up the stacker. (ST-02). When hooking the chain up, EE felt a pull in the right shoulder. This EE had not received any work restriction that affected the job at the mine site, but did have restrictions. EE was released as of 9/5/2017. Disputed EE's claim & now moved forward with shoulder surgery. Completed on 4/19/18. No lost time or modified duty till surgery.

EE was removing an oil filter from a track mobile engine and later that day ee felt pain in left elbow. This was considered a first aid case however on 08/02/2017 ee received a medical procedure that made this incident MSHA reportable.

Employee was attempting to remove a bushing from a sheave positioned on the ground. While repositioning to its side the sheave began to slip and the employee got right index finger caught between the sheave and adjacent structural steel support. The right index finger was pinched and caused a laceration that required 10 stitches. EE was released to returned to work.

While cleaning a ditch at front gate the employee must have tweaked right knee. Employee is unsure what happened & when it happened, there was sudden pain, leg just started hurting. Employee has arthritis in right knee that rubbed together and damaged the meniscus with a flap tear. Employee had out-patient surgery on 5/19/2017 to repair meniscus and they removed arthritis.

The employee was carrying a Screen eccentric on shoulder, while taking it off shoulder to load in the back of a pickup truck ee smashed middle finger on right hand between the tailgate and eccentric.

Employee was shoveling sand at the Utica II crusher to improve housekeeping and reported pain in lower back at the end of shift. This was originally classified as a first aid case only, however on 1/13/2017 employee received therapy that made this case MSHA reportable. Employee did not miss work and continued to do normal assigned job duties.

On Saturday, November 12th 2016, EE was replacing a motor at approximately 8:30 am. The motor and the iron created a pinch point and caught EE's gloved pinky finger between the two. EE reported the injury to supervisor, and they immediately were seen at the clinic at Spruce Pine. EE was examined and given stitches.
