The warehouse manager was lifting a 12" bearing from a transportation cart to the storage area. While lifting EE felt a pop in EE's right bicep.

A mechanic was moving 55-gallon barrels by hand. The barrels were loaded with solid materials. When EE rolled a barrel into place, EE lacerated and broke EE's left middle finger at the distal phalange when it came into contact with another 55-gallon barrel.

Employee was starting a piece of equipment, EE reached around to unlatch door the point of the antenna struck employee on the side of the nose and slid up under EE's safety glasses and poked EE in the corner of the eye. Employee was prescribed an antibiotic eye drop to prevent infection.

While reinstalling a shovel tooth the employee's finger was smashed between the tooth and the adapter. The tip of right ring finger was split open and needed stitches.

The bulk grease tank could only be raised out of the containment approximately 1 foot due to configuration of the pump hoses and piping. Once at that height, the pump could be removed, allowing the grease tank to be removed from containment and clean-up to proceed. The employee bent over to remove pump and felt sharp pain in back.

Slaven Crusher screen plant had built up wet material on the inside of the chute that collapsed onto the belt and plugged it. The employee was shoveling from an awkward position and strained his shoulder while cleaning the belt. The employee was seen by a physician who prescribed physical therapy and lifting restrictions of 15 lbs.

THE EE WAS REMOVING AN OXYGEN BOTTLE FROM THE RACK, WHICH HAD A 20" SNOW DRIFT IN FRONT OF IT.THE BOTTLE SHIFTED AS HE REMOVED IT, CAUSING HIMTO TWIST HIS KNEE. THE EE DID NOT SEEK MEDICAL ATTENTION UNTIL 1-04-02. AT THAT TIME IT WAS DETERMINED THERE WAS TORN CARTILAGE. SURGERY IS TENTATIVELY SCHEDULED FOR 1-10-02.
