An employee was installing a spray bar on a wash screen. EE dropped the spray bar on EE's right index finger while attempting to bolt down the spray bar. It fractured the tip and caused a cut requiring approximately 6 stitches on EE's right index finger. Two other employees were on the other side of the screener, but neither visually witnessed the accident.

An employee completed repairs on a conveyor belt. When employee released the belt clamp it sprung and hit the end of employee's index finger and fractured the bone at the end of the finger. The employee is back to work with restrictions and there was no lost time to employee as we paid the employee the full wages for the time employee missed going to doctor.

While performing maintenance and wearing a welding helmet, employee cut the top of his head on a plastic scraper on a screener requiring 8 stitches.

EE WAS MOVING PIECES OF STEEL AND CUT HIS RIGHT WRIST. HE PUT A BAND AID ON AND KEPT WORKING. LATER IN THE DAY HE WENT TO A DOCTOR WHO PUT IN 7 STITCHES. NO LOST TIME FROM WORK. HEALING FINE.
