A contractor employee, (NBS Industrial Services LLC - MSHA ID #C6519), was performing clean up duties. The employee was pushing a shovel when EE's hand began to burn and EE was unable to fully move EE's right wrist. The employee was taken to the ER where EE was examined and told to follow up with an orthopedic.

The injured employee was throwing away scrap metal (metal, rollers, etc.) into a dumpster. The employee had the truck parked next to the dumpster. When they went to throw something in, they said they "felt a pop in their back". The employee was taken to the hospital for examination. The doctor stated it was back spasms.

The injured employee was assisting on stemming a shot by raking material from an excavator bucket into the holes. EE claims EE's right shoulder popped. EE continued working until the job was complete. EE then got in a haul truck to move material and said while EE was being loaded, the truck shook and EE's shoulder began hurting more.

The injured employee was assisting to unstop the box above the 5 1/2 crusher. The material shifted and a rock spilled out of the box striking the employee on the hard hat and shoulder. The employee was taken to the hospital where they were examined. It was determined the employee had a concussion and shoulder contusion.

Employee was working on a flatbed trailer in the garage. Employee was removing the air spring, it fell, employee reached to catch it, left ring finger was pinched between the air spring and a piece of metal. Cut/broke finger.

EE began EE's shift at 3:30 p.m. on 1/11/16. Performing normal duties under normal conditions. EE started out shrink wrapping material. Then EE was going to begin loading rock dust into bags. EE bent over to pick up an empty bag, when EE said EE felt a pop/pull in EE's groin.
