The incident occurred on 12/1 but employee didn't report it until close of shift on 12/2. Employee was clearing a head pulley when they pulled a rock out another fell smashing their left pinky causing a fracture. Our employee was wearing gloves.

Showing all 13Employee stated they were pulling electrical wire and slipped and fell on a rock landing on right shoulder. Employee was pulling wire for a new construction crushing plant. Employee said they were not injured. Employee worked the rest of the day. Employee works half a day on Wednesday July 9th and complains of shoulder pain.
On 11/10, EE told supervisor EE was having abdominal pain, thought it was a personal health issue. On 11/15, doctor diagnosed with a groin strain and gave restrictions. EE claims the injury occurred while handling worn shoes on the VSI crusher on 11/09. Worn shoes weigh approx. 50 lbs.
Employee had removed cover guard to put oil in reservoir. EE set the guard beside the reservoir and after adding the oil, EE and another employee went to put the guard back into place. After lifting it up in tandem they turned to the right to align when employee felt a sharp pain in their lower back.
While changing out bucket pins on excavator one pin fell against bucket and struck mechanic on left foot standing approximately 5ft from bucket. The pin struck steel toe of shoe and rolled back onto the foot. The pin weighs 80 - 100 pounds. Risk assessment was conducted before removing pins. Fractured 2nd and 3rd toe
Pulling the hose up a hill, pain in stomach.
Employee was changing out broken bolts in the clamping cylinder on the Cone Crusher, as employee went to align the cylinder block to start the 1st bolt the block twisted and mashed the employee's ring finger on left hand. The finger received a laceration behind the finger nail requiring stitches.
Installing suction hose on Metso HP400 crusher. Used a band saw to cut the hose, held the hose with left hand, used a small grinder to dress up the end of the hose. EE lost grip on the hose and one of the small wires punctured glove and the tip of finger. Became infected, sought medical treatment on April 7, antibiotics were prescribed to fight the infection.
Working on a bearing of a tail pulley he was in the process of repositioning the tail pulley to access the bearing on the left side. After the block was removed the pulley became lodged above the conveyor frame on the mounting bolts. He attempted to free the pulley using his hands when the pulley shifted and caught the pinky finger of his right hand.
EE was releasing come-a-long from a conveyor that he had tied off while making repairs to the belting. Upon releasing the latch the cable hung, ee jerked the come-a-long with his right hand. The latch released pinching his right thumb. He went to the urgent treatment clinic and they gave him a tetanus shot and placed 2 stitches in the side of his thumb.
EE said he was stepping up into the water truck and felt a muscle pull in his leg. EE said he was using 3 points of contact when this happened.
Employee was lifting flat bar metal from the back of a pickup truck. He said his shoulder started hurting after that.
Miner removed radiator cap to check fluid level, when hot liquid splased on right arm and side.
