Removing grease lid from barrel. EE cut right index finger on edge of grease barrel. The barrel was on crusher service truck. EE received five stitches on right index finger.
Unbolting chute while sliding it into place it slipped pinning hand between chute and hoper below. Put bandage on hand & went back to work. EE called me Sunday 3/25/12 to tell me he was going to have his hand x-rayed. I had no notification he was having problems till Sunday.
He was working at the pit line area, flinging 2x4 wood boards out of the way. A splinter from one of the boards entered the underside skin of his left hand middle finger. He removed splinter right away on 7/7/09, but on 7/16/09 it looked infected so he went to doctor on 7/16/09, after work. It was cleaned & antibiotic prescribed.

EE REPORTED HE WAS CLEANING UNDER JAW CRUSHER CONVEYOR WITH HAND SHOVEL. TWISTED WRONG & FELT SHARP PAIN IN GROIN AREA. EE DID NOT REPORT INCIDENT TO SUPERVISOR UNTIL MAY 9, 2000 WHEN EE WENT TO THE DOCTOR. SEE ATTACHED.
WAS CHANGING SCREEN ON SECO FINISH SCREEN MIDDLEDECK GRABBED UPPER FRAM TO PUSH SCREEN OUT WITH FEET WHEN HE FELAT A "POP" IN FOREARM
EMPLOYEE WAS ON HANDS AND KNEES CLEANING UNDER THE TAIL PULLEY OF THE CRUSHER LIKE HE DOES EVERY MORNING.GRABBED A ROCK TO THROW OUT AND SHARP PAIN STARTED IN GROIN AND RIGHT SIDE.
