Employee was removing cable clips between a 40 mm and 3/4 cable the 3/4 cable was broke and was frayed {had cable wires sticking out} while removing cable clips it allowed the 3/4 cable to slide down and cut their wrist.

Showing all 3A piece of steel with a lifting eye welded on top of it was sitting in the upright position for lifting. The employee attempted to lay the piece of steel over to keep it from possibly falling over and mashing someone's foot. When EE did this the glove became caught on the lifting lug and EE was unable to get hand away.
Miner was conducting crusher maintenance - moving a piece of the crusher to a different location - while setting the piece down, crushed left ring fingertip.
