employee standing in confined area shoveling material off a conveyor. Employee was hunched over shoveling and twisting the shovel over his back to remove material. Strained lower back
Slide shaker screen into place by twisting body when felt burn in lower back; waited for another employee to help. not much discomfort that day but next morning lower back/hip were stiff. walked around and it got better. Monday morning very stiff and painful to walk so made doctors appt for Tuesday. Tuesday filled out paperwork and went to ER and doctor's appt.
Employee while performing repairs was attempting to steady a hydraulic cylinder end for removal (7" by 4" thick, approximately 10-15lbs). The cylinder end came loose and fell a distance of approximately 6 inches onto employee's right hand. Employee received minor cuts and bruising on right hand.
Employee was positioning a piece of steel plate inside of a rock box so that it could be welded into place, when it slipped and mashed his right thumb.
