Showing all 5Removing a brass bushing off main shaft of crusher, bushing had oil on it. Another co-worker was helping lift bushing off but hand slipped causing bushing to fall and caught employee's fingers under bushing causing bruising to end of index, middle and ring fingers on right hand.
Employee was changing a light bulb in the control panel at our pug mill plant. The bulb broke causing a laceration to right index finger tip. Laceration required stitches.
