Miner, along with the assistance of other miners, was pulling on the ground, an uncharged electric cord near the end of the shift. Later that night and next morning, EE felt discomfort which they attributed to this task. EE went to primary doctor that following day who ordered an ultrasound which the miner had on 8/12/2020.

Working on level ground & pulling on chain - dislocated shoulder in which miner was examined & arm placed in a sling. Miner perhaps pulled at an awkward angle. Chain was 3/4" thick & 16' long.

INJURED WAS GREASING END LOADER WHEN HE BUMPED HIS HEAD ON LOADER BUCKET ARM. HE LATER DEVELOPED PAIN IN UPPER BACK & AFTER TAKING TREATMENTS FROM DR, HIS DR DETERMINED THAT INJURY WAS FROM PR EVIOUS ACCIDENT. AT THAT TIME, THE OFFICE WAS INFORMED.
