An employee (EE) was working on a haul truck. They removed the access plate located under the windshield of the operator cab, and placed it on the catwalk leaning on the guardrails. When the EE dismounted, they heard something hit the ground behind them and saw a shadow. The EE raised their arms and the panel struck the EE's shoulder and then their head.

Showing all 382 mechanics were reinstalling a plant crusher accumulator. The part had 3 sheared bolts and broken seal causing a hyd oil leak. EE was lifting the part back into position by hand while waiting for the hyd jack to hold it in place. Accumulator got in a bind with the support clamp and fell back to the deck smashing EE's left middle finger. EE was wearing gloves. Part weighs 30-40lbs.
Employee was assisting to move a battery box. EE had opened the lid of the box and braced it to check the forklift fork position. While looking into the box to check fork position, the box shifted and the lid fell down striking the left hand. This resulted in a two inch laceration that required stitches. EE was taken to the emergency room and Quarry safety personnel were notified.
Mechanic was servicing a bearing cylinder & shaft-approx weight 50lbs. It was secured in a vice that was loosened to reposition the bearing. The bearing slipped. Mechanic tried to catch it causing the EE's right thumb to get caught between the vice stand & bearing. This resulted in lacerations & a broke bone. EE was taken to the ER and treated/ released.
Employee was removing a pressure switch, when Employee released switch, a plastic part of the switch flew and hit EE's right middle finger.
While employee was servicing chains & opening doors, left pinky finger got stuck in the doors.
Employee was checking engine oil dipstick, felt sharp pain in lower right side of back.
Worker was picking metal out of concrete feed. Felt something in left eye.
Worker was trying to remove water canon hose. Hose wire cut thumb.
Employee was sorting punch plate screen panels. Panel was dropped on uneven ground which caused it to bounce back, hitting employee on the shin.
Trying to line up the bobcat tire to the studs when it slipped pinching right middle finger and between the stud and rim.
Lifted a rock to move it out of the way. Hurt back.
Fitting in new adapter for bucket, strained back.
Unloading a leaf spring and the item bounced up when he dropped it on the ground. Spring pinched the end of his finger.
Attempted to lift and reposition drill steel that had fallen out of storage carousel. Steel slipped downward while in grasp.
Changing hammer on crusher, fell back on finger.
Employee was removing a rock which was stuck in the cone crusher. Rock was too heavy and while still holding rock, had slid down side of crusher and smashed left hand, middle finger.

Removing rubbish from king bin, rubbish bin had a gauge on top. While looking in bin sliced left palm on top of bin.
Employee was flipping the gate on S-9. After removing the nut on the Waianae side, employee was pulling the bolt out with his right hand and catching the 2 washers with his left hand. The gate fell down, crushing his finger between the handle and angle iron.
While concluding loading blast, employee took a step away from the bore hole he had just loaded and his left knee had two popping sounds and buckled with pain.

Cleaning head pulley chute and was removing a chunk of mud when finger was punctured on a worn out piece of metal.
Loaded boxes onto truck while preparing for blasting. Employee did not report injury until 11/23/05.
THREW SHOVEL ON BELT, AS CLIMBING UP THE END OF THE SHOVEL, IT HIT HIM IN THE LEFT EYE. SWOLLEN ABOVE LEFT EYE. LEFT EYE RED.
ADJUSTING TAIL PULLEY ROLLER, HAMMER GOT WEDGED BETWEEN ROLLER AND BELT AND RIGHT ARM, SHOULDER AND SIDE OF BODY GOT SUCKED UP INTO ROLLER. SEVERE ABRASIONS AND BURNS TO RIGHT ARM, ELBOW, SHOULDER AND SIDE OF BODY SCRAPED. ALSO SCRAPES ON LEFT KNUCKLE.
WHILE CHANGING SCREEN, ONE OF THE SHARP METAL ON THE SCREEN PUNCTURED RIGHT HAND (BACKSIDE) BETWEEN THUMB AND INDEX FINGER, PULLED OUT OLD SCREEN. EE IS ON LIGHT DUTY UNTIL NEXT APPOINTMENT WITH THE PHYSICIAN ON 12-24-02.
WHILE OPENING FRONT GATE, GATE CATCH PINCHED AND SMASHED FINGER.
WHILE WORKING ON IMPACTORS (CRUSHER) ON B-PLANT,AREA WAS TOO SMALL AND HE WORKED IN AN AWKWARD POSITION.HE STRAINED HIS LOWER BACK. EE CONTINUED TO WORK THROUGH 10/22/02. STAYED HOME 10-23 & 10-24-02. RET ON 10-25-02
WHILE WELDING RAILING SUATTING DOWN, OTHER WELDER WAS MOVING/SETTING ANGLE IRON TO BE WELDED AND ACCIDENTALLY HIT INJURED'S HEAD WHILE INJURED WAS WELDING.
MOVING ROCK OUT OF WAY OF DRILL, PULLED SOMETHING IN RIGHT SIDE GROIN AREA.
HELPING WELDER TO LIFT STEEL PLATE ONTO CONVEYER BELT, RIGHT PINKY FINGER WAS SMASHED. FINGER HAD SWELLING AND CONTUSION.

EXPERINECED NAGGING PAIN IN LEFT SHOULDER WHEN MOVING AND NOT MOVING. **EE REPORTED INJURY ON AUGUST 12, 2000 (SATURDAY) TO SUPERVISOR, CALLED HIM AT HOME. EE WAS ON VACATION FROM AUGUST 8, 2000 THROUGH AUGUST 20, 2000.
UNSURE, MAY BE PULLING WATER HOSES, DRIVING AND EVERYDAY ACTIVITIES; HAS BEEN ONGOING SINCE LATE JUNE. EE FEELS NUMBNESS, TINGLING, AN ARTHRITIC FEELING IN THE LEFT HAND.
WHILE LOADING, CARRYING & DISTRIBUTING ANFO BAGS, EE FEELS PAIN, PINS & NEEDLES SENSATION IN HIS LEFT SIDE FINGERS, SHOULDER AND LEGS. **EE FINALLY SOUGHT MEDICAL ATTENTION FOR INJURY ON JUNE 1, 2000. THUS FAR, HAS LOST ONLY THIS DAY AS TIME AWAY FROM WORK.**
LIFTED A 5 GALLON BUCKET OF WATER TO CLEAN TRUCK, RE-INJURED RIGHT AND LEFT SHOULDERS.
LIFTING AND CARRYING A 30 LB BEARING TO SET INTO HOUSING OF PRESS, EXPERIENCED SHARP PAIN IN UPPER BACK AND AROUND RIB CAGE AREA.
