I,NAME WAS PREP5RING TO ROOF BOLT AND WIRE THERIBS ALONG SIDES OF THE MINE SHAFT UPON THE EIGHT LEVEL, HE WAS SETTING UP HIS ROCK DRILL AND OILER TOGETHER ON A WORK DECK, HE CLIMBED DOWN THE SHAFT TO RETRE3VE SOME OTHER TOOLS, DOWN APPROX. 10 FT HE WAS STANDING WHILE HIS ROCK DRILL OILER FELL DOWN AND STRUCK HIM ON HIS RIGHT HAND, FINGER (INDEX WAS FRACTURED).
