Showing all 44At approximately 6:15 pm, EE was working on the platform at M280 head pulley area, during the replacement of M281 tail pulley. While removing compartments and metal plates to access the work area, one of the plates became stuck, as EE attempted to free the plate, the metal plate suddenly came loose and struck the inside of EE's left knee, causing a laceration.
When placing the hose on the connection, the end of the hose that connects to the water hose hit employee.
EE was working / cleaning around M216 conveyor area with a water hose, when suddenly and unexpectedly EE felt a pain on EE's left knee.
Working on loader assembling the center hitch, was setting rear frame on to jack stands 25 ton with 3' tubes, was adjusting the tube that sets the height, as they pulled pin that adjusts this left hand ring finger was caught in the hole of the jack stand, as tube slid down it slid onto the finger caught in the jack stand hole.
While working on M20 conveyer, a rock bounced of the empty conveyer impacting the employees hard hat.
Employee threw welding connector, looks like a clamp, it bounced back at their nose.

While working on the DL the EE had water drip onto forehead. The water ran under EE's safety glasses and into eye. When the EE woke up the next morning the eye was irritated. The EE was taken to the DR. EE's eye was flushed out. DR prescribed antibiotic eye drops. EE returned to work.
EE was working on M-11 using the pressure washing truck, as EE stepped down from the truck EE's fingers got stuck between the handrail and taillight causing a laceration of the small finger in the right hand.
While hosing off excess material from a portable crusher a piece of foreign matter entered employee's eye.

Open fracture of L-hand middle finger distal phalanx (tip) resulted after screen shaker pulley being replaced by IE slipped and fell over IE's injured body part. IE is being treated by physician. He is working under restricted duty (no use of L-hand). Contributing cause: IE failed to use proper mechanical devices (come-a-long) to hold pulley in place while being replaced.
EE was on a conveyor belt that slide down, which caused him to twist and he injuring his ankle.
EE WAS ATTEMPTING TO REINSTALL AN AIR CONDITIONING VENT IN EE LOADER CAB WHEN EE FINGER WAS CAUGHT BY THE FAN

A WATER HOSE WHICH HAD JUST BEEN TURNED ON SLIPPED FROM THE HANDS OF AN EMPLOYEE AND THE WATER STRUCK VICTIM IN THE EYE.
EMPLOYEE WAS TURNING THE ROTOR SHAFT IN ORDER TO CHANGE THE HAMMERS. WHEN THE ROTOR SHAFT MOVED FURTHER THAN INTENDED HE TRIED TO GET OUT OF THE WAY, BRUISING HIS RIGHT LEG AND DISLOCATING HIS LEFT SHOULDER. NOTE: SHOULDER HAD BEEN DISLOCATED PREVIOUSLY. DOCTOR SAID SHOULDER WAS UNSTABLE PRIOR TO ACCIDENT.
CLAMP ON HOSE CUT HIS HAND WHEN THE HOSE SLIPPED FROM HIS HAND.
THUMB WAS CAUGHT BETWEEN A ROCK AND THE TIRE OF THE STACKER.
SAFETY LATCH ON THE RAIL CAR DOOR RELEASED, CATCHING THE FINGER BETWEEN THE LATCHES AND THE RAIL CAR DOOR.
WHEN THE SHAKER WAS BEING REMOVED FROM THE ROAD ROCK CRUSHER , EMPLOYEE'S LEFT HAND WAS PINNED BETWEEN THE SHAKER AND A STEEL BEAM.
AFTER COMPLETING THE WELDING TASK EMPLOYEE FED THE EXCESS WIRE BACK THROUGH THE WIRE FEED MACHINE ONTO THE WIRE REEL ASSEMBLY, EMPLOYEE'S FINGERWAS CAUGHT IN THE WIRE FEED GEARS.
