Employee was climbing out from changing a screen in Tower 1. Employee was wearing gloves and PPE and caught a sharp piece of screen with their hand, cutting it through the glove.

The employee was shoveling under a conveyor inside a tunnel. Lifting material onto belt with the same shove. On light duty. Cleared to drive EE's truck. Physical therapy. Employee hoped the injury would get better. Waited until 10/2/2020 to have it checked out even though it happened 9/28/2020.

Employee was changing wire screens. Lifting the screen into place strained arm and finger. The quarry management was not notified until 7/25/2019. The employee was hoping it would get better. Sent to doctor and was placed on light duty and physical therapy follow up appointment.

Two EEs were taking a fuel tank off a pavement roller. There was pressure in the pump that sprayed when the line was taken off. Both EEs were wearing safety glasses. The EE did not notice irritation until EE woke up the following day (Saturday). Sent EE to be checked Monday morning. Urgent care gave EE prescription eye drops. No lost work time.

The employee was filling a 777F with diesel fuel at the start of the shift. Strained right arm while doing so. The employee went back to full work hoping it would get better. We were informed on 10/5/2018 at the end of the shift. The employee wanted to see if it got better with rest. The employee asked to go to the doctor the following week. Physical therapy assigned 10/25/2018.

The employee bent over to pick up a metal chute door in tower 1 of the screening plant. When the employee came to me ee looked to be in pain and was having trouble moving. Took employee to an urgent care where the doctor prescribed a muscle relaxer at the employees request. Put on light duty and went back to work on the same day.

Shoveling out the rock and grease accumulation around the bucket boom of a 992K and felt discomfort in forearm. Still had movement. Sent for examination. First doctor returned employee to work with no PT. At a check up a second doctor recommended PT on 4/20/2017 with sessions starting 4/21/2017. At work on light duty.

Employee (EE) was discarding a conveyor idler when finger got caught in idler, pulling right arm/shoulder. EE reported injury but did not seek treatment. EE sought treatment on 10/2/2015 as shoulder soreness did not improve. Diagnosed with unspecified shoulder strain, and ordered physical therapy. Returned to duty w/no restrictions on 10/2/2015.
