EE was loading 18" culverts onto a trailer when one became unstable. EE tried to stop and reload it back onto the trailer. EE felt a pop on the inside of right elbow. EE suffered a torn distal biceps tendon.

EE was housekeeping below a screw case. EE bend down to clean and hit their head on the frame holding the case up.

On Sunday 04/18/2021 the employee informed the supervisor that they were experiencing numbness and wanted to go to the doctor on 04/19/2021.
Employee had completed a welding project on the hopper. The employee was cleaning up the work area and packing tools out of the area. As the emplyee was lifting a 5 gallon bucket full of tools up onto the service cart, they felt a sharp pain in their right shoulder. Employee is going to seek orthopedic examination at this time.
While working with metal, performing maintenance duties, the worker cut their hand while trying to catch a falling piece of metal.

The employee was installing heater pipes when they struck their shoulder against an upright pipe flange. The employee went to the doctor a couple weeks later due to the pain they were experiencing, and on 12/27/2018, quit working.

X-rays at the hospital revealed his hand was broken in a couple of places. He is scheduled to see a surgeon soon for plan of action.
Employee Threw his back out stacking bags. Felt spasms in lower back, Lost most function in his left arm from pain in the back. Is receiving chiropractic treatments and has not missed any time off from work.

The plant was shut down when EE went to install B58 belt on classifier fan. He pulled on belt and assistant pushed, causing his right pinky finger to run between belt and sheave. Finger was broken and bleeding due torn tissue. He was taken to specialist in Powell, Wyoming for successful surgery. Though serious, this injury was not life threatening.
EE lifted a cutting edge to install on a 627G scraper and twisted lower back. He felt pain then and for the next few days before going to a Chiropractor on 06/03/2013 who told him bone spurs prevented proper alignment of the back. He is going to another doctor today, 06/10/2013 for further diagnostics. He is currently off work.
EE was stacking 100 lb bags when he felt a pain in his left arm. He went to Urgent Care where they diagnosed him with a tear in his left bicep. He is undergoing therapy and rest at this time.

EE was involved in palletizing 100 pound bags when he spun bag around to get a better grip. This action caused trauma to right hand area and thumb was sprained and possible ligament damage but doctor is not sure. He will return to work on light duty on 07/03/2011 for about 6 weeks tentatively.
EE was barring track onto dozer, felt discomfort in lower stomach. He thought it was pulled muscle and waited a few days to see if it got better - it did not. He went to doctor on 02/27/2011 to confirm it was a hernia which will require surgery. No lost time as yet.
EE lifted a 100 pound bag with the help of his left leg when he experienced a pull in his muscle followed by some pain. He went to doctor and took a few days off to heal. He was diagnosed with a pulled groin muscle and was released to work on 01/04/2010.

Trauma was done to the trunk of the body, most likely resulting in a hernia. The doctor has not yet been consulted and no time has been missed as of this point; on self imposed lite duty restriction.
