Employee was opening a metal box of new welding rods and went to reposition the box and their body to fully remove the lid. While doing so the box tipped over and as it was tipping employee caught it. The box weighs 50 pounds, and the lid is thin sharp metal, the edge of the lid made contact with employees left wrist while being caught and caused a laceration.

Fueler rubbed their right eye 2/6/26 with work gloves on. The discomfort started 2/7/2026 and continued to 2/8/26 and the lower right eyelid became noticeably irritated and tender. Medic described the injury as a chemical eyelid exposure with a possible infected eye duct. The employee was prescribed antibiotics.

Mechanic was lifting a tire and wheel off a tire machine and felt a pop sensation in their lower right abdomen. Employee went to the clinic for medical evaluation as needed then reported back to work. The employee was seen by their primary care physician on December 3rd for additional evaluation. It was then determined that surgery would be needed to repair the injury.

At this time the employee started removing the lead from the light and then that's when a big piece of dried up lead fell from above the light fixture and landed on EE's left thumb, that EE had holding onto the ladder with.

Sep 29-Oct 2: Evaluation. Oct 2-16: R&R. Oct 13: Doctor notes surgery needed, light duty. Oct 17-: Planning Bay Office.

While performing a normal task that involved pulling a hose using a specific tool ( Fork). The task required prolonged physical effort over the course of several hours, a blister developed on the operators right hand from repeated pulling motion. The blister became infected, requiring treatment from the onsite clinic with antibiotics.

While changing out light fixture on the fuel island. The light was positioned in a way that the light was not illuminating the ground. Once one bolt was loose, they proceeded to loosen the second bolt, and that's when the fixture containing the light pivoted in a downward manner, smashing the employee's right middle finger.

Two Mechanics were rebuilding a pump. While breaking the pump down to rebuild it, one mechanic was removing a socket from a cordless impact gun (the mechanic was wearing latex gloves and not cut resistant gloves). When the socket came off, the mechanics hand struck a sharp piece of piping resulting in a laceration to their right hand on knuckle.

While mechanic was manually repositioning a lip guard in a wooden shipping box, the lip guard slid pinching employees pinky finger between the side wall of the box. The employee immediately stopped the job and reported the injury. Employee reported to the clinic for evaluation and received several stitches to manage the injury.

Worker was taking apart a poly spray gun over a bucket to contain product that might leak out. The worker isolated the valves to the Polyshield A and B components and proceeded to disconnect the spray gun. As the worker removed the spray gun, product splashed up from the bucket, into the workers eye. Worker's PAPR was in the up position. Product splashed under their safety glasses.

Employee was unchaining a drill rig in the front while the contractors unchained the back. The employee was on the top deck of the trailer under the raised drop gate of the drill rig. As the operator was removing themselves from under the drill rig it began to slowly tip towards them. The drill rig slowly came to rest on the employee's leg pinning it to the top deck.

Employee was perfoming a pre-trip inspection of a light vehicle they closed the driver door on their finger causing a laceration to the right index finger. Employee stated the grip they used to close the door resulted in them not moving their finger out of the way in time.

Employee slid a bare hand across the box flap contacting a 3" staple that entered the bottom and exited the top of the left-hand index finger. Staple was removed by the PA, and the employee returned to work without any restrictions.

While removing locking bar spacer blocks from the filter press employee caught respirator on a hydraulic hose then slipped and fell. Employee was holding a large wrench which smashed fingers between the wrench and the pan doors of the filter press.

Employee was repairing a multi-leaf overhead door. Using the manual operating handle to raise the door to add counterweights, attempted to disengage and revert back into the motor drive position. The door started to lower and the handle rotated and struck the employee in the chest.

Fire technicians were cleaning out a strainer while conducting maintenance on a diesel fire pump. While in the process of reinstalling the cover one of the technicians lost grip and control of the cover, which resulted in the fingers of the other fire technician being pinched between the cover and the strainer casing.

Worker removed their PPE-safety glasses while entering the break room at break time and felt something enter their eye. Worker flushed eye and it still felt irritated. Safety Supervisor escorted worker to the onsite medic. Medic flushed the eye again, removed a FB and gave the worker antibiotic eye drops for preventative measures.

Worker was handling panel at grade on work benches. Was attempting to knock snow and ice off a panel by repeatedly picking up and dropping it on the work bench. The wind caught the panel and worker tried to dump it off the bench. It twisted and one corner popped up and contacted cheek, resulting in a minor laceration requiring sutures.

Mechanics were removing bolts from transmission bellhousing in equipment yard. At approximately 3:45 pm. Foreman came to rear of dozer to check on I.P's progress and noted that IE had the transmission ready to secure to prevent slippage. The foreman and 3rd mechanic were preparing rigging to secure the transmission when the transmission slipped and came to rest on I.P's left leg.

On October 21, 2018, Port Operator was shoveling lead/zinc concentrate spillage underneath the P28 conveyor belt. On October 23, 2018 the employee notified the Port Medic of a sore back. The employee was sent to the Mine for evaluation by the PA. The employee was sent off-site for further medical evaluation.
