Miner working on emission analysis equipment located in the third stage of the pyro processing tower, Miner was cleaning the sample probe, the sample probe was still hot when the miner accidentally touched it with EE's hands. Miner show minor burns in palm of EE's hands.

While EE was welding a plate inside the crusher, a rock that was lodged in the rollers (overhead) became dislodged and struck EE in the back of the head, near EE's ear. Medical treatment was provided and EE was diagnosed with a concussion.

EE was cleaning a chute and it appears EE stuck hand into a clay bailey while holding a flash light. As hand entered through the clay bailey and into the screw conveyor housing, a piece of round stock cut hand.

Employee was holding a ladder for a co-worker while ee was installing a ballast cover for a light fixture. While the co-worker was adjusting the east side so a bulb would fit the west side came loose causing the cover to fall and strike and cut employee's left hand/wrist.

EE was running a vactor truck and the hose became plugged. In an effort to unplug the vactor hose he hit it against a wall. Dusty material on the wall fell down and got into the ee's eyes.

Miner was installing a motor when he hurt his back at aprox. 10:30 am on Thursday the 6/7/12. He did not report it until Monday 6/11/12. He thought it would just go away. He was sent to the Dr. on the 11th after reporting the injury. He was released to full duty work on that day. 7/3/12 he asked to go back to the Dr. as his back was still hurting him and he was given light duty.

Employee was working on electrial project. He was pulling on a large bundle of wiring when he felt a sharp pain in his lower back area. Subsequent diagnosis indicated that EE suffered lower lumbar strain.

Maintenance shop mechanic was installing new handrail section on haul truck. As he was pushing and pulling hand rail into mounting holes, he felt a pain on his left side. He at first denied medical treatment, but reported that injury was not any better on 11/5/09. Further diagnosis indicated that EE had torn some rib cartledge. EE is on modified duty.

Accident happened at 42BE02 while removeable take up support bar was in the process of being removed. His finger got pinched between bar support holes in elevator housing. EE received medical treatment, returned to work the next day.

The employee was shortening the rigging on a 400 lb. metal drum. It came loose from the forklift and fell, striking the employee's right knee, sliding down both shins, and stopping on the steel toe boots.

Employee was removing exhaust manifold off the engine of 85 ton Wabco haul truck. The turbo slipped and caught the left middle finger between the manifold and steering arm causing a fracture of the finger tip.

EE's were working with 2 chainfalls to remove a damper gate. One chain hung up and the EE put his hand where the gate would come down. When the gate came down, it pinched the right index finger.

EE was removing a strip of "T" rubber caught in the lift conveyor of the sweeper truck. When pulling, tension on the conveyor was released and one of the conveyor flights caught EE. Laceration to forehead, R-eye orbit fracture

EE reports that he was lifting a section of 1/2" plate steel with 18" hole cut out and clay bailey attached using a rope. Approx. weight 40#. The following morning, upon awakening, experienced distended hemorrhoid. Causal relationship being investigated.

EE reported injury on 6/3/05. EE had propped a dust collector (81DC17) cover against the guardrail to exchange bags. A gust of wind caught the cover and it tipped over striking EE on left shoulder. Possible bone separation at tendon attachment to L. shoulder.

EE WAS OPENING A HATCH ON ROTARY FEEDER FOR 53CM02 COKE MILL WHEN HE LOST HIS GRIP ON THE DOOR WITH HIS RIGHT HAND. THE DOOR (HINGED ON BOTTOM) DROPPED OPEN, PINCHING EE'S LEFT HAND AGAINST THE HOUSING. RESULTED IN HAIRLINE FRACTURE TO LEFT INDEX FINGER. (FINGER DID NOT HEAL; SURGICAL IMPLANT OF PIN REQUIRED.)

Employee was cleaning up material in the vicinity of the primary crusher most of the workday. At or near the end of the shift, the employee experienced lower back discomfort. Six (6) days later, he reported the incident and completed a report, but declined medical treatment. Two (2) more days passed before he elected to seek medical attention (Dec. 02).

Employee was opening the outer door on the man-material elevator when the bottom lip of the top section struck his right elbow. The outer door is in two sections that are mechanically connected so that they open together. Employee was pushing the bottom section down with his foot and the upper section struck his right elbow as it passed his arm braced on the open inside door.

A truck driver working for V-Express was standing in the waiting room for silo #10 with his back to the hinge side of an outwardly opening door that was partially open. Another driver pushed the door closed pinching the driver's right third finger tip between the door and frame. The injured driver was not an employee of National Cement Company, Lebec Cement Plant.
