THE EMPLOYEE WAS INSTRUCTED TO REMOVE SEVERAL BAGS OF CEMENT (100LB BAGS) FROM THE BACK OF A PICKUP TRUCK NO DISCOMFORT WAS NOTICED IMMEDIAT ELY BUT HE DEVELOPED A BACK ACHE THE NEXT MORNING H E NOTIFIED ME THE FOLLOWING MON MORNING BUT STA TED THE PAIN WAS MINOR I NOTICED HE WAS STILL SORE A MONTH LATER & TOLD HIM TO GO TO A DOC WHO DIAGNOSED A HERNIATED DISK
