Employee had been moving 12" x 12" blocking with a partner. After finishing job employee was riding in a truck and began to feel pain in right inner thigh. Was seen by NP and returned to work. On 8-15-16 employee returned from one week of vacation and complained of increased pain. Seen by MD on that date and put on restricted duty.

EE states he had back pain after working with a partner to lift a small barrel of lube from the floor onto a portable cart. He refused treatment initially but upon wakening the next day, he requested an evaluation. He was evaluated by a medical provider and taken off of work.

Two employees were removing an inner race from a primary mill reducer shaft. During removal of the inner race, a piece of the race chipped off and became lodged in the left inner forearm of an employee. EE was seen by an EMT and site nurse and transferred to the ED for further treatment.

EE was working on a spray bar and while attempting to remove the spray bar he injured his left pinky finger. He was evaluated by a Physician, diagnosed with a fracture and sutures closed the wound. He returned to work.

EE was working on 5 conveyor discharge chute replacement. During demolition of the chute, a piece of the chute dropped down and pinched the left hand resulting in a suturable laceration to his left index finger. He was evaluated by a Physician & nine sutures closed the wound and the ee returned to work.

EE was carrying a 2' long piece of 2 x 4 macro bar. He bumped his head which caused him to lose his grip on the bar causing it to fall. It struck the employee on the top of the foot. He was evaluated by a Physician and given a release to return to work with restrictions.

Our worker was cleaning a restroom and as she was connecting a sprayer (that had a chemical called Century Q in it) to a garden hose the connection wasn't secure spraying back on her face even with safety glasses on the water and chemical ran down her forehead and into her left eye.

Employee attempted to latch a dry basket in the up position in the women's locker room at the end of shift. She thought it was latched, but when she let go the basket fell and struck her in the head. She was evaluated by a physician and sutures were administered to close the wound. She was scheduled off on 7/4. She returned to work full duty for her next scheduled shift on 7/5.

Employee alleges that she struck her right knee on a cyclone structure at work around 8:45am on 12/19/13. The incident was first reported to supervision around 4:00pm on 12/19/13. She was evaluated by a healthcare provider on 12/20/13 and released back to work with restrictions.

EE was removing trash from an office waste basket that seemed to be struck. She yanked the plastic bag, freeing it. Then the bag and the contents quickly came up. Unable to control this action her left wrist struck the corner of the metal desk chipping the wrist bone.

Working on an engine compartment, employee felt debris enter right eye. Was evaluated by an ER physician on 6/8 and given a referral to eye doctor on 6/10. Employee didn't report for his 6/9 shift due to eye complications. He was evaluated by the eye doctor on 6/10; a foreign object was removed from eye, Rx drops were issued, & the doctor took him off of work for the date of 6/9.

Employee was scheduled off on 12/29/12-1/1/13. Due to pain and pain medications, she could not work on 1/2/13. She was evaluated by a physician again on 1/3/12 and was taken off of work at this time.

While rolling a clamp over to a pick-up site, the clamp rolled on uneven ground and fell onto the EE's right foot. The EE was evaluated by a physician on 8/30/12 and was able to continue working full duty. In January 2014 the EE complained of non-resolving right foot pain. He was evaluated by a physican and surgery was performed on 3/13/14.

Employee was handling a 13.8 V cable (that was partially coiled), while picking up the coil- he walked over a berm and twisted the cable to align the pins in order to plug in the cable. He felt pain in his lower back while doing this task. He was evaluated by a Physician on 5-12-12 and taken off of work. He returned to duty on 5-17-12.

EE was working on a cobber feed pump. In the process of changing the liners, a pallet had to be moved closer to the forklift. The suction liner slid onto the left hand, trapping it between the liner and the pallet. The ee pulled his hand free and a short time later, he felt back pain. He has been evaluated by a Physician and a steroid injection was ordered & admin on 12-16-11.
