THE EMPLOYEE REPORTED TO SUPERVISOR THAT HE THOUGHT HE WAS HAVING BACK SPASMS WHICH HE SAID HE HAD A HISTORY OF AND WANTED TO GO TO THE DOCTOR. HE MISSED 1 DAYS WORK AT DOCTORS ADVICE AND GIVE N MEDICINE TO RELIEVE DISCOMFORT. EMPLOYER DID NOT KNOW UNTIL 10-28-91 THAT THIS WAS WORK RELATED AS STATED ON MEDICAL BILL COPY.
