Showing all 5Worker was attempting to weld on a piece of c-channel iron placed on a welding table. When leaning over they knocked the iron off the table and onto their right foot. The impact broke 2 bones in the arch of their foot.
Four employees were carrying a steel pipe. Two of them dropped one end of the pipe and the pipe landed on one of their feet. The impact broke the miner's right pinky toe. The miner was wearing steel toe boots.
Employee was conducting maintenance on a dozer's tension idler. The idler was unknowingly under pressure and when the mechanic began removing the bolts holding the idler in place the idler broke loose and struck the mechanic. The idler fractured the knuckle on his right pinky finger and the area of the skull above his right eye.
EE was conducting maintenance on the truck scale. A trucker drove onto the scale despite EE telling the trucker to stop and wait for him to finish. The scale shifted slightly and smashed EE's finger, causing the right ring finger to break.
