He was replacing a conveyor belt yesterday when a piece of metal came loose and smacked his hand. He sustained a laceration to his right index finger.

Original date he felt his knee pop when he kneeled. Had it looked at by drs and MRI. Dr found nothing but told patient that he could do exploratory surgery if it did not get better. This was not a reportable accident until 7/9/14, when surgery was done. Dr removed scar tissue and a pillia--outpatient surgery and able to report back to work his next working day.
