EE and another ee saw big rock in pile that shouldn't be there. We're inspecting screen panel condition & discharge cluck flop gate position. Both men were on opposite sides of catwalk. EE put his hand inside chute and the other ee pulled a lever on the other side. The gate was closed on ee thumb. He received 5 stitches on his thumb and lost his nail.

Showing all 7Employee was docking the dredge for the winter when one of the winch cables got caught underneath the corner of the dredge. Lenny tried to pull the cable free when he strained his shoulder. He didn't think anything of it until weeks later when the pain continued. He went to the doctor on 11-9-2012 and at that point was restricted from work.
Miner was researching part for part number and part came undone from hose and sprayed miner with 10-W hydraulic oil. Miner flushed immediately with saline and as a precaution was taken to a local medical facility. No damage and miner was released to full duty immediately.
Employee was replacing the blade on the d6 dozer when he bumped the hydraulic cylinder with his shoulder causing the cylinder to fall on and cut his left thumb. He severed 2 tendons and required surgery to repair tendons. Follow up appt. is 4/11/08 followed by physical therapy. Released for light duty w/restrictions.
Operator was servicing his crane. He bent over & slightly twisted his body while trying to lift a 5 gallon oil container. He strained his lower back.
PERFORMING ROUTINE MAINTENANCE ON CEDAR-RAPIDS CRUSHER, BUMPED RIGHT ELBOW ON STEEL FRAMING.
EE WAS REMOVING SWING CLUTCHES FROM CRANE. ONE OF CLUTCH HALVES BECAME STUCK. HE REPOSITIONED HAND TO LOOSEN. CLUTCH HALF LOOSENED BEFORE HAND WAS REMOVED, THEREFORE CRUSHING FINGER.
