Employee was blowing product off a Pnuematic truck when line plugged up. In the process of investigating the plugged line, employee got LKD in their eyes. We immediately took the employee to the ER where they flushed EE's eyes.

Showing all 50Injured worker was installing a variable frequency drive. Injured worker attempted to place a bar underneath and then fell and struck their knees on the concrete. Injured worker continued to work after incident without restrictions. After physical therapy injured worker had surgery on September 6, 2024, to repair left meniscus tear and has now missed work.
Employee was unplugging the LKD blow line for kiln #5. When employee dumped LKD on the ground, employee was standing in the dump pile and the hot lime was burning EE's foot.
Lime dust particles blew in the right eye of employee while employee was sampling lime from kiln #6
The miner was mixing up mortar in a bucket. When the mixing was complete the miner lifted the bucket and felt a muscle pull in lower back.
Employee was replacing a motor mount. While trying to align the motor to the new motor base, the employee got a finger hung in a bolt hole in the motor base cutting.
Miner attempted to move forks of a fork lift. Miners gloved hand slipped off the fork and little finger of left hand was caught between heel of fork and beam below bar where fork hangs from.
Miner cleaning and door slammed on EE's right middle finger.
Accident happened in equipment yard when jumper cables slipped from miner's hand making contact with miner's face area creating a gash near lip.
Employee removed glasses to clean hands and face with vinegar. While washing his hands, vinegar splashed into his right eye.
Miner was climbing into the cab of the truck and hit his right knee under the second rung of the mounted truck ladder. Reported to immediate supervisor, however medical attention was not warranted per Miner request. Miner went to the doctor on his own behalf on 09/29/2014,the employer became aware on 10/07/2014 when notified by Miner that medical attention had been sought.
The miner was rolling up his welding leads when lime got in his right eye irritating it.
Employee was replacing buckets on a material elevator, lime contacted his skin around at various locations including his eyes. Employee received 1st and 2nd degree burns. Employee missed 2 scheduled shifts while under physicians care to prevent infection.
Employee was clearing a blockage in transfer chute of tunnel screw that was carrying hydrate into a collecting screw. Upon clearing the blockage, hydrate was forced from the access port onto exposed body parts causing lime burns to the head and facial area.
Employee dropped a steel I-beam on right foot breaking bone in great toe. Employee was placed on restricted activity. At follow up evaluation on 2/24, restrictions were extended and the company can no longer accommodate the restrictions placed upon the employee by the treating physician. Lost time as of 2/24/14.
Employee was helping another employee washout a hydrate tank with a water hose when some hydrate lime got in his right eye. He did not think that he needed to go to the doctor until 9-14-13.
While fabricating & installing a chute, employee bumped his knee on the adjustment bolt. He did not feel this to be a significant incident at the time. However, in the evening of 8-13-13 he sought medical treatment at an urgent care center and was diagnosed with cellulitis.
A bag of hydrated lime was traveling up the conveyor sideways. The employee reached to turn the bag when it struck the bag flattener and blew up causing lime to go in his eyes and throat.
Employee was pulling the Guzzler hose out of the Cooler Pit and up the stairs. He has restrictions that prevent him from working and also has Physical Therapy for the next 2 weeks.
Employee was cleaning up in the Hydrate Bag area when a bag of lime broke and caused some lime to get in his right eye.This became a lost time on 4/28/11 with restrictions that prevented him from doing his job. He got a full release on 4/29 and worked thru 5/2 until the Dr put the restrictions back. He is again lost time as of 5/3/11
The Employee was hand bagging on 9-B-3 when a bag came off the belt and broke. Lime went into his eyes.
Employee was moving rock knockers to be placed on a forklift. One of them slipped and mashed & cut his left index finger requiring stitches.
Employee was attempting to remove buildup in preheater. He was using a piece of pipe about 3 ft and smashed his finger in the process.
The employee was straightening a hydrate bag that had jammed when it opened and blew lime into the employee's right eye. Has received medicated eye drops and missed one day of work.
Employee went into the tunnel to clear clods off of grates. When he stuck his hand in, he cut it on a piece of metal. He received stitches on his thumb and index finger.
The employee was moving a roll of shrink wrap from a pallet to the forklift. He has had soreness but did not ask to see a doctor until 9/22/09. Did not become Lost Time until 9/23/09.
The employee woke up sore on 5-1-09. He had been pulling brush from the front of the Dozer. Asked to see a DR on 5-4-09. He does not know when injury happened. He does not have any restrictions that prevent him from doing his regular job, however he has multiple visits to a Chiropractor scheduled.
The employee was adjusting a bag of hydrate in the palletizer and pinned his right ankle against a beam and cut his leg requiring 7 stitches. NO LOST TIME, however he had work restrictions for 2 workdays.
While using the power washer EE noticed that the water tank was running over. He pulled the unit out from the wall and attempted to bend the float shaft down. His hand slipped cutting open the top of his left hand on the stainless steel tank. NO LOST TIME however he did receive stitches and medical treatment.
Employee was rebuilding screen drive. He was removing the bearing and housing from the drive when it popped off striking his left wrist. NO LOST TIME, however he does have some work restrictions.
Employee was changing clutch on auxiliary engine of Guzzler Truck when he caught his knee on a sharp edge of the toolbox. No Lost Time or restricted work duty - Medical Only due to receiving stitches.
While opening the hood on the Cat Haul Truck, the employee pulled or twisted his back.
When the bag fell off the Bagger it had too much air in it and hydrate lime spewed out of the bag when it hit the conveyor. Lime went under the employee's safety glasses into his left eye. Employee went to the E.R. on Saturday afternoon.
Employee was cleaning fines out of a screen hopper with a pipe. Some fines came down and the pipe struck him on the side of the head. Employee continued working until 11-10-04 when he requested to see a Dr. An MRI was performed on 11-11-04 to rule out a concussion. The MRI was considered normal and the employee was released to return to work full duty on 11-12-04.
Employee was cleaning out discharge chute of 3-BC-1 when the head pulley caught the metal bar, slinging it into the right jaw of the employee.
Employee was cleaning out the hydrate tunnels when a pile of lime fell down and flew up into his face.. He had lime burns to both eyes and corneal abrasion of the right eye. He received antobiotic drops and missed one full shift of work.
Employee said his back hurt after stacking 50# bags in the Hydrate Bagging Dept. Asked to see a doctor about 9:00 AM.
EE IS A HYDRATE BAGGER/STACKER. HE ASKED TO SEE A DOCTOR ON 10-30-03 SAYING THAT HIS SHOULDER HURT. NO SPECIFIC INCIDENT HAD OCCURRED. HE TOLD THE DR. INJURY HAPPENED ON 10-16-03, HOWEVER NOTH ING WAS REPORTED UNTIL 10-30-03. NO LOST TIME-PHYSICAL THERAPY AND RESTRICTED WORK DUTIES ONLY.
EE WAS REMOVING CAP FROM ROTARY VALVE ON BLOWER PIPE WHEN IT BLEW OFF & HIT HIM IN THE FINGER.
EE WAS USING A SHOVEL TO CLEAN UP MATERIAL IN THE SCREEN HOUSE. INJURY WAS NOT REPORTED UNTIL 6-17-02. NO LOST TIME. HOWEVER, HE HAS SOME WORK RESTRICTIONS AND WILL BE RECEIVING PHYSICAL THERA PY.
P-LANT TRUCK HAD RUN OUT OF GAS. EE WAS PRIMING CARBURETOR WHEN IT BACKFIRED AND BURNED EE.
EE WAS RAKING CLODS ON THE KILN FLOOR AND STRAINED LOWER BACK. EE IS RECEIVING PHYSICAL THERAPY AND HAS SOME RESTRICTED WORK ACTIVITY- NO LOST TIME.
LOADER OPERATOR DROVE UNDER PLATFORM WHERE EMPLOYEE WAS WORKING TO REMOVE WASTE MATERIAL. AS MATERIAL WAS BEING REMOVED STEAM AND QUIKLIME ROSE UP CAUSING LIME BURNS TO EMPLOYEE. NO LOST TIME. RESTRICTED WORK DAYS ONLY.
EE WAS LOADING AN OXYGEN BOTTLE WHEN THE CAP CAME OFF AND HIT HIM ON THE CHIN. THIS INJURY REQUIRED MEDICAL ATTENTION WHICH INCLUDED THREE STITCHES. NO LOST TIME OR WORK RESTRICTIONS.
THE SCREW SHAFT HAD BROKEN & EE WAS REMOVING LIDTO CLEAN OUT FOR REPAIRS. HE HIT HIS ELBOW ON THE LID. HE DID NOT ASK TO SEE A DOCTOR UNTIL 4/24& WAS SEEN AGAIN ON 5/1 AT WHICH TIME THE DOCTOR RECOMMENDED PHYSICAL THERAPY FOR 4 WEEKS. NO LOST TIME - MEDICAL TREATMENT ONLY.
EMPLOYEE WAS REBUILDING A SET OF STAIRS. HE WAS TRYING TO LINE UP THE CHANNEL IRON BY HAND WHEN HE INJURED THE LEFT RIB AREA OF HIS BODY. THE INJURY WAS REPORTED ON 3-14-01 AND HE SAW A DOCTOR ON 3-15-01. THE COMPANY DEEMED THIS AS FIRST AIDCASE BECAUSE THERE WAS NO MEDICAL TREATMENT, OTHER THAN DIAGNOSTIC, NO LOST TIME AND NO WORK RESTRICTION.
EE WAS PULLING UP WELDING LEAD. THE ROPE BROKE AND HE TRIED TO GRAB THE LEAD BEFORE IT FELL TO THE GROUND. HE DID NOT ASK TO SEE A DR UNTIL 2-12-01. HE HAD NO LOST TIME MEDICAL TREATMENT OR WO RK RESTRICTIONS UNTIL 7-30-01. WHEN EE HAD OUT PATIENT SURGERY SHOULDER ARTHROSCOPY.
MOVING A SEPERATOR FAN BLADE, PULLED HIS BACK & BRUISED HIS FOOT.
EMPLOYEE WAS PUSHING UP WASTE LIME IN THE REJECTPIT AND GOT LIME IN HIS RIGHT EYE SOMETIME DURING THE NIGHT OF 7/13/00. HE DID NOT REPORT INCIDENT UNTIL 7/15/00. HE WAS SENT TO DOCTOR 7/17/00.
