Employee states that this was during the morning work hours but he does not remember the exact time. Employee did not report this as a workman's comp until 10 am on 4/17/2008.

Employee was cleaning perlite with a shovel and was putting it into a wheel barrow.

EMPLOYEE WAS TAKING A FINISHED BAG OF PRODUCT TO WAREHOUSE, BAG STARTED TO FALL OFF FORKLIFT & EE TRIED TO STRAIGHTEN BAG ON PALLET. EE PULLED A MUSCLE ON THE LEFT SIDE OF HIS ABDOMEN RESULTING IN A HERNIA.

Employee pulled a muscle in his lower back while performing his regular duties. The exact cause is unknown, however, he had been sweeping,shoveling, operating a back hoe, and around 3:30 p.m. he helped another employee lift a 1/4" plate of steel from the saw horses. Later that evening, when he was exiting his truck at home his back was hurting.

EE WAS PUTTING A NEW BELT IN THE #3 ELEVATOR. WHEN HE TRIED TO TURN BELT AROUND AS IT WAS GOING INTO THE ELEVATOR, HE PULLED SOMETHING ON HIS LEFT SIDE OF GROIN AREA.

EE WAS OPENING THE DUMP GATES ON BOTTOM OF LOADED WASTE TRAILER. EXERTED EXCESSIVE FORCE WHEN PULLING LEVER TO OPEN THE GATE, & EE FEELS HE MIGHT HAVE PULLED OR STRAINED A MUSCLE. ***REPORT OR IGINALLY NOT COMPLETED OR FILED WITHIN THE REQUIRED TIME FRAME; BECAUSE THE EE DID NOT SEEK MEDICAL ATTENTION.**
