EE was inspecting dust transport screw. The system went positive as the employee was turning to close the screw inspection port cover back on, covering the employee in bypass dust. Employee suffered chemical burns to both eyes.

Employee was installing a 6" water pipe. As the pipe was set into place, both index fingers became caught between the pipe and the stand, causing fractures to both fingers.
Right index finger caught between steel members during rigging operations. Pinch point. Worker was wearing gloves. Improper hand placement. Sustained laceration requiring sutures to close. Returned to work without restriction.
Employee was removing jackhammer from catwalk approx 6ft high. As the employee pulled the jackhammer down to the ground level, the jackhammer oiler struck the employee in the mouth, causing a chipped tooth.

EE was attempting to climb onto an equipment(TA-40). Weather conditions were icy, wet, and cold. As he attempted to climb on equipment his foot slipped on the stirrup and he caught himself on the rails to prevent him from falling. He heard something pop in his right shoulder.
Employee was returning from getting water in Maintenance Office. Upon re-entering the loader cab he put his right hand where door closes and utilized his left hand to shut the door. He thought he could move his right hand before the door shut. The door shut on his right middle finger. He reopened the door and removed his finger.
EE was assisting another ee in shoveling material from a hopper that had stopped up with material. As he shoveled material he felt pain in his right shoulder.
Employee was building chute and cut their right forearm on piece of sharp angle iron.
Employee was moving metal disk weighing about 300 lbs by rolling it on edge. The disk fell over and struck employee on outside of left foot. Sustained "Jones fracture" to fifth metatarsal.
Employee was attempting to use magnetic block to reset switch on BMH reclaimer, when reclaimer was moved by another employee and caught employee's finger between sensing block and rail block. Right fingernail evulsion and laceration. Employee treated and released. NO LOST TIME.
Employee was pushign brick on conveyor. He trapped his right index finger between two bricks and sustained a crush/pinch injury to the finger tip and nail.
EMPLOYEE DROPPED A 5-GALLON BUCKET CONTAINING SAMPLE DISCARDS THAT WEIGHED ABOUT 50 POUNDS. HE TRIED TO CATCH THE BUCKET BEFORE IT HIT THE FLOOR AND IN DOING SO STRAINED HIS LOWER BACK. LATER IN THE DAY HE REQUESTED MEDICAL EXAMINATION. TREATING PHYSICIAN DIAGNOSED A LUMBAR STRAIN AND PRESCRIBED MEDS AND PHYS THERAPY.
A cooler had broken down. They were passing 5 gallon buckets full of material from person to person. EE said he grabbed a bucket and turned too hard to hand it to the next guy. He felt a pain in his right groin area.
EE WAS CLEANING TOWER. WHEN HOT BUILD UP PIECES CAME BACK OUT DOOR & GOT IN HIS EYE.
Employee was installing overhead grate and stretched a muscle in his upper back. He was placed on restricted duty for three days.
Employee was working in raw mill with dusty work environment, did not notice any problem until he woke up the next day with irritated right eye. Employee was examined by clinic physician and a dust particle was removed from the surface of the eye.
Employee was attempting to slide a metal panel when the corner slipped and pinched his fingertip. He reacted by pulling his finger away which caused a laceration on the fingertip. The laceration required two sutures to close.

Employee claims he was getting set up to cut wood on the welding table in the maintenance shop. When he set circular saw on welding table, an 8' x 2', 1/2" thick piece of steel that was sitting on the welding table fell on the top of his left foot. The piece of steel was hanging off welding table and the weight of the saw was enough to drop steel off table.

EE IS STILL UNDERGOING CARE WITH ORTHOPEDIC SPECIALIST.
Working on a piece of rebar, trying to bend the rebar & employee's rt. leg moved & twisted ankle.
Bent over to pick up jackhammer & bit. When he raised up he felt a sharp pain in his side.
He was working on the tower using a jack hammer and a bar. the bar was inside the door on the tower and he reached for the jackhammer to put it on the bar and he felt a pain in his back.

LINE 1 SCRUBBER TOWER WAS PROPERLY ISOLATED FROM PROCESS GAS (KILN EXHAUST GAS) IN PREPARATION FOR OPENING THE MANDOORS TO VISUALLY INSPECT THE DEMISTING PADS FROM THE OUTSIDE OF THE VESSEL. THERE WAS QUITE A BIT OF STEAM COMING FROM THE MANDOOR. THE UPPER DAMPER WAS OPENED IN A EFFORT TO CREATE A DRAFT AT THE DOOR AND CLEAR THE STEAM, SUFFERED ASTHMA ATTACK.
