The Employee was changing a starter in a Cat 777 haul truck, when the starter fell from its Position on to the employee's right thumb. The thumb was caught between the starter and a support, smashing it.

The employee was assisting a mechanic in blocking equipment from movement. The employee was on his knees moving a 6x6 24" long (about 15lbs). He picked the blocking up less than a foot from the ground, and felt pain in his back. The pain, at times, went down his legs. Employee transferred himself home.

The employee was replacing blocking material under the portable plant structure. The 6 x 6 x 4ft wooden blocking was being set in place when it slipped from the employees hands and fell down. When it fell, the employee did not have time to move his hand and the blocking fell onto the end of his left ring finger.

The employee was using a grinder with a cut-off wheel, to cut a piece of allthread. The employee had a nut on the allthread to use as a guide and the allthread was secured in a vise. The employee had one hand on the grinder and the other hand on the allthread. He was using a saw motion to cut through the piece and the grinder jumped off the piece onto his hand causing a cut.

The E. was attempting to replace a trough roller structure on a conveyor. The E. placed the roller structure onto the conveyor frame and was attempting to put it into the upright position, between the belting and frame, when his thumb was "wedged" between the two. The E. experienced trama to his thumb and drove himself to the E.R..

The employee was attempting to connect 2 sections of pipe together. A pipe that was beinng attached to a permanent section of pipe had resistance to it because of size and weight. The couplers were put together, but before they could be secured (locked) they came apart. A coupler struck the face of the employee. He sustained a cut below his lip requiring stitches (4).

While clearing material that became "jammed" from the discharge of the crusher, a rock fell onto the conveyor onto the employee's left ring finger. The rock caused a cut requiring stitches and broke the bone in his finger.

THE EE WENT TO THE ER ON 3-18-01 WITH BACK PAIN. DR. GAVE MEDS AND TOLD TO FOLLOWUP W/PERSONAL DR. THE EE CALLED OFF 3-19 & 3-20 AND FOLLOWED UP W/WORKERS' COMP. DR ON 3-21-01. WORKERS' COMP DR SAYS POSSIBLE HERNIATED

WHILE THE EE WAS GREASING UNDER HIS ROCK TRUCK DEBRIS FELL MUD ON TO HIM GETTING BEHIND HIS GLASSES AND INTO HIS EYE. EE FINISHED HIS SHIFT WENT TO ER NEXT DAY. DR REQUIRED HIM TO BE OFF WORK 2 DAYS.
