Employee was trouble shooting a plugged line. When the washout came free it sprayed hot liquid onto the employee resulting in 1st and 2nd degree burns on their forearm, neck and upper chest.

The employee dropped a 5-foot vertical section of scaffold tubing on self when the employee hooked the rosette on an I-beam while trying to position it. The I-Beam pulled the tube off the pin of the other vertical section employee was using to gain height to check clearance.

Employee was lifting a 4 in vacuum hose and felt sharp pain in right arm. Employee was taken for evaluation and it was determined that the tendon between the forearm and bicep had a tear. Employee had surgery to repair.

Employee was helping change the inner boom of the miner CM02. While guiding the hoses that run through the boom, the boom shifted from the front causing the boom to strike the employee's right thumb between the machine stops and the boom itself. Employee was evaluated and taken to town to receive stitches.

On 12-21 employees were changing valve. Valve slipped striking employee in the shoulder area. Employee was taken to doctor and evaluated and was released to return to work. Employee later experienced pain and went to see a specialist on 1-30-2018. Employer was notified on 1-30-2018 that the employee required physical therapy and would not be able to work until completed.

Employee was working on fiber cast line got something in his eye, rinsed their eye out. Eye felt fine. Two days later, employee felt discomfort in their right eye. Was taken to the dr. for evaluation. Had to see eye dr. Foreign object was removed and employee was given antibiotic drops. Was released back to work with no restrictions.

Employee was removing a site glass on a shuttle car hydraulic tank. When the nut broke free their right forearm came into contact with the opening on the hydraulic tank. Resulting in a laceration. Employee was transported to local hospital and laceration was sealed shut with dermabond glue. Employee was released to return to work with no restrictions.

Employee was using a porta power and wood block to move a piston cup. The wood block slipped hitting a pry bar that was not being used and causing the pry to strike the employee in the hand. The employee was taken to the doctor and it was determine that the employee needed sutures.

Employee was cleaning out dust tube, the door for the access to the tube slipped out of the employees hand falling and hitting the employee on the head, causing a cut. The employee was transported to doctor's office for observation and it was determined to require stitches, the employee received the stitches and was released.

Maintenance Tech was changing out a 4 inch valve. He was holding the valve and stabbing bolts. He felt a pain in his left bicep, he was checked out by a first aid leader and transported to Sweetwater Memorial Hospital for observation. He was determined to have a hurt arm and he was given lifting restrictions.

Employee was turning condensate water valve, reaching with arm above water filter canister. As employee turned valve on, canister cover blew off, striking employee's arm. Employee was treated at hospital for burn and buildup of air/fluids in upper arm. EE was able to return to work full duty.

The EE was working in the coal storage area that day and had no complaints. EE went to work next day and was in the control room when he felt something in his right eye. EE and supervisior washed out his right eye, but could not get the coal out of it.

EE was going up a silo in a man basket. As he reached the work level he prepared to put on his blast hood. He noticed his rope grab was sitting on the basket & his life line had not gone through. As he started to feed the life line through it whipped back & hit him in the right eye. The strike ruptured blood vessels and tore his retina. He was not wearing his safety glasses.

While retrieving 3" water pipe from cont miner (CM01) using lift truck, pipe hung up on J-hook & fell into lift truck. Pipe bounced when it hit & struck ee's hand. EE rec'd sutures in right middle finger & ring finger. Ligaments in middle finger were torn.

Employee picked up ore sample in 5 gal bucket to bring to Lab. Bucket was fuller than usual & was very heavy. Employee tried to use one hand, but couldn't lift the bucket. Then EE used both hands to lift bucket. A day later, employee felt pain in shoulder. **Employee returned to regular duty. On May 22, he was unable to work. Dr determined it to be result of 4/22/07 incident.**

Another employee opened up top door and inundated employee below with rush of air & dust. He received medical treatment for dust in eyes.
