Was doing maintenance on the crusher. Taking the drive chain off the hopper and the guard tipped over and hit the top of head and hurt neck. EE was wearing a hard hat.

Employee was replacing track pad on the excavator and when he went to put it on it slipped down and fell on both feet. He broke his left foot and a toe on his right foot.

Employee was adjusting the position of an iron chute on a pair of sawhorses when the sawhorse moved and the chute fell against the employee's knee. The weight caused his knee to give out.

EE WAS LIFTING A SCRAPPER FROM A SAND JUMP BELT WHEN HE PULLED SOME MUSCLES IN HIS BACK. HE DID NOT FEEL THE PULL AND WORKED THE REMAINDER OF HIS SHIFT. HIS BACK TIGHTENED UP THAT NIGHT AND TH AT WAS WHEN HE WAS AWARE THAT HE HAD HURT IT.

AFTER CHECKING OUT MACHINE, THE EE SHUT THE ENGINE COVER DOOR, BUT IT DID NOT SHUT TIGHTLY. SO HE OPENED IT AND SHUT IT AGAIN, BUT DID NOT GET HIS LT INDEX FINGER OUT OF THE WAY OF THE DOOR, & IT SHUT ON IT BREAKING THE END OF IT.
