Employee inspecting shotcrete coverage on new air door install, & as EE grabbed the west de-energized air door, while pulling it open to inspect behind it, in doing so the the direction of air flow was east to west causing the east door to close resulting in EE's right hand to be pinched between the doors.

Employee reports was changing steering valve assembly (approx 50 lbs). Set unit on mucker hood to continue work. It fell off the hood contacting employee's right pointer finger resting on mucker frame, causing a blow out type injury and fracturing the tip.

The employee reported that while assisting to re-position a steel wedge EE was contacted on the leg by the wedge. The contact caused a laceration to EE's thigh requiring sutures to close.

Employee reported to the supervisor at the end of shift that EE's left shoulder is hurting, denies any event to cause pain, stated was only doing normal work activities no trauma, stated just normal wear and tear. Seen site medic, referred to ER, Then onto Occupational Health Provider where MRI was scheduled and employee was restricted from full duty.

Employee reports a piece of collar pipe contacted EE's eye while EE was installing the pipe in a lifter-hole. The contact required surgery to repair the damage caused to the eye. Employee reports was wearing safety glasses at the time of the incident and the piece of pipe came around the glasses.

A tech. expert on site to advise during mill re-line was injured when EE operated a liner trolley with a liner on the cart. The cart stopped causing the liner to rock and fall. The injured reached to stop it but liner fell on EE's arm breaking forearm bones. Out of an abundance of caution following a conversation with MSHA we are reporting on behalf of the Canadian Company PolyCorp

employee was prepping a diamond drill rig for a move and in the process of loading the drill rods into the storage rack one rod slid back resulting in pinching EE's right hand between the rib and end of the drill rod resulting in a crush type injury

LOTOTO employee took guards off a pump EE was working on. After removing all required bolts to remove the impeller, EE realized needed to move the pump a bit for the impeller to come out. Employee used their hands & grabbed the pulley belts to turn the pump. In doing so, pulled EE's right thumb thru the pulley resulted in fracture and laceration.

Employee reports utilizing a "micky" bolt to bend hog wire in preparation to hang in the back. Employee reports pushing down on the bolt to bend the wire when the bolt slipped backwards contacting their bicep and causing a laceration. Employee was taken to ER to have injury cared for.

Employee had stopped mucking & noticed the radiator cap looked loose on the loader & decided to try and tighten it. In doing so, the cap blew off & hot coolant contacted EE's left arm and residual splatter to EE's face. Seen site medic, referred to ER, restrictions issued for mild 2nd degree burns.

EE was attempting to move a rock that had fallen off one of the rail cars that had just been loaded when the rock slipped out of their grip resulting in the rock to fall back against the rail car pinching EE's left ring finger between the rock & the rail car. Restrictions were issued at the ER for fracture and amputation of finger tip.

Employee walking on a sidewalk from where they parked vehicle, with a backpack & caring a case to the admin. building when they felt worsening pain in lower right back that has been an ongoing issue that they has been seeking care for prior to this incident. Seen site medic, referred to ER, x-rays negative, referred to next level of care where restrictions were issued.

During installation of an I-beam in an existing structure the beam became stuck, in efforts to free the beam employee had their right hand at end of beam. When it dislodged it moved forward, resulting in the employee's right pinky finger being smashed.

After loading a shotcrete pot into a light vehicle, employee closed the tailgate & in the process pinched right ring finger between the tailgate & shotcrete pot resulting in a laceration which required 3 sutures. EE was given a full work release making this a medical reportable injury.

Employee was undoing a bungie strap holding materials in place when it slipped out of their hand contacting EE in the mouth resulting in a upper front tooth being chipped. Seen dentist and was given full release on 9/11/23, however had tooth repaired on 9/25/23 making this a medical reportable as no days were lost.

On 6/26/23 employee was attempting to lift a stand up computer desk when they felt a pop in left shoulder. It was reported to the operator on 6/27/23 at the beginning of the shift, was evaluated by site medic and referred to ER. X-rays were negative, lifting restrictions were issued making this a medical reportable incident.

Employee was attempting to lift a wire spool onto the jack stand without help. When EE lifted the spool, EE saw their right bicep move up the arm. EE felt discomfort but no pain. EE stopped and reported to on-site supervisor. EE was transported to the hospital and seen by a doctor and diagnosed with a ruptured bicep.

The core innertube hung up in the top of the drill rods on a +25 up-hole. The Helper placed a hand on the innertube to shake it and when it came free, their right hand fingers were crushed between the top of the innertube latch head and the sheave wheel bracket on the rig frame.

An employee was working from the platform of a scissor deck installing pipe hanger brackets. As they were adjusting the length of the metal bracket by spinning it off an expansion shell when it fell out of the hole. The employee attempted to push themselves away from the falling bracket. The bracket struck the top of their hand, piercing their glove.

Miner hanging 6" HDP airline pipe in the 85-655 development ramp. Caught left ring finger between pipe and cab rail of Young scissor deck USD04 causing laceration requiring sutures and fingertip fracture.

An employee was in the bolting cycle using a CMAC bolter and in the process of picking up an eight foot threaded rebar bolt, EE felt a pop in the right shoulder followed by pain. Seen site medic, referred to ER, then to orthopedic where restrictions were issued at that time.

The employee states was helping to stand a booster pump up when EE felt pain in the left hip. EE was seen by on site medic and taken to the ER. EE was referred to ortho and an MRI was conducted on 9/12/2022 and was diagnosed with a herniated disk in the back and kept off work.

Employee stepped off of the bolter & identified a hazard. EE had a rock in the travel path, so EE picked it up & was placing it down by the rib. It rolled pinching EE's right gloved middle finger resulting in a laceration. Site medic referred EE to ER. Two sutures were required, full release was issued making this a medical reportable.

IE was manually adding an HM38 drill rod to the shank. EE knocked it against the drill to remove mud in the end and smashed the right ring finger between the drill rod and the drill resulting in a fractured fingertip (tuft) and open wound.

Employee setting up to bolt, put section of wire screen in the back using a mickey bolt tapped in by hand & other end was held up by a scaling bar. EE bent over to get 2nd mickey as EE stood up 1st mickey fell out allowing screen to fall contacting EE's face resulting in laceration to right eyebrow & cheek, sutures required. Full work release issued making this medical reportable.

Employee along with partner were in the process of closing a chute draw bridge, when the employee's partner pulled on the chain to close the draw bridge it pinched the employee's right gloved pinky finger between the draw bridge and the hand rail they had it resting on resulting in a avulsion and fracture to the right pinky finger.

Employee was cleaning a muck spill on the crusher floor using a high pressure water hose, when the nozzle assembly separated from the hose. As EE went to shut the main water line valve off the water, the hose whipped contacting the right thumb, resulting in a fracture to the tip of EE's thumb.

When they went to manually push an 8 foot threaded rebar bolt into a resign filled hole so they could get the bolt onto the driver they felt a pop in left shoulder followed by mild pain. Seen by site medic, referred to ER, restrictions were issued with a referral to an orthopedic specialist.

Employee had strung water hose out to wet down a muckpile & when they went back to turn the header valve on they grabbed the valve on the air header instead of the water and when they opened it fully the coiled up air hose connected to the valve whipped contacting them in the left cheek area causing a laceration & fracture.

On 7/14/20 employee was putting EE's hardhat on when the cap lamp battery came unhooked from the back of the hard hat swinging around and contacting EE's right upper incisor tooth chipping it. Employee continued to work EE's shifts, while on days off EE had the tooth repaired on 7/28/20 making it a medical reportable incident at this time

Employee along with EE's partner were loading 20' sections of 4" aqua (PVC) pipe onto a flatbed for transport. While lifting a section of the pipe EE's partner lost grip & dropped the end of the pipe causing the other end to bounce up & contact the employees upper right incisor tooth. Seen by dentist for chip full release issued, making this reportable.

Employee drilling with a jackleg & EE changed from a 4' drill steel to a 6' steel it slipped out of EE's hand & contacted EE's right foot. EE continued to work & when EE's foot started to hurt EE reported incident, was seen by site medic, referred to Er, X-rays positive for closed hairline fracture in right little toe, full release issued, making incident a medical reportable.

On 5/13/20 EE called in saying back was too sore to work, at that time was seen by an orthopedic specialist & given restrictions making incident reportable.

While Employee was unloading 8 x 8 x 12 wood timbers off a flatbed truck for a paste wall, EE had one of the timbers roll onto EE's gloved right index finger smashing the tip area of finger. Seen by site medic, referred to ER, x-rays positive for fracture and surgery was required.
