Showing all 13EE was climbing stairs on the Allis screen catwalk when he felt and heard a pop in his right knee. He stated that he was carrying a bucket with 12 bolts within (approx. 15 Ilbs) in his right hand and two wrenches in his left hand while he was climbing the stairs.
The employee was in the process of removing a large rock that had fallen down on the inside of the cone and caused cone not to start. Had to remove it and while lifting out the rock the rock slipped in his hands, dropping on left hand.
Installing a steel liner in the chute of the simplicity screen, the liner had a few small areas of steel slag along the edge. The employee tried to position the liner into place but was unsuccessfull. He then tried to position it by stepping on the suspended liner but in doing this caused the liner to shift, pinching his right leg between the liner and chute. Saw doctor 12/22/06.
EE & co-worker were changing hammers on impactor. They'd finished 1 set & were rolling impactor to do other set. Co-worker rolled impactor around, wheel started to free wheel to its heavy side, co-worker lost his balance & fell on wheel. EE reached in & pulled him back to keep him from falling. As EE was holding co-worker, impactor stopped & spun the other way catching EE's knee.
EE exited his equip. to assist a customer operat- ing a dump truck clean off the tailgate of his dump truck. The customer was prying on the tailgate to open it slightly when a rock shifted falling onto employees right index finger.
Ee did not dismount the screen conveyor properly using three-points of contact. The ee jumped approx 4 to 5 feet to the ground causing a contusion to his left foot (heel). The ee working with him did use 3-points of contact and was fine. The ee was given a written warning. Prior to that morning a safety talk was given on 3 pt contact.
EE WAS ATTEMPTING TO UNLOAD SEVERAL SCREENS OUT OF A LOADED BUCKET ONE AT A TIME, ONTO A FLATBED TRAILER. WHEN HE PULLED THE TOP SCREEN OFF THEY ALL CAME OUT AND STRUCK HIM ON THE RT. KNEE.
DRILL STEEL COUPLER FELL BEHIND DUST COLLECTOR CUP. EE GRABBED THE COUPLER AND MOVE THE SLIDE LEVER UP AND THE COUPLER CAME UP SMASHING HIS LEFT FINGER BETWEEN COUPLER AND I-BEAM MAST.
EE WAS WORKING ON AN IMPACTOR WHEN HE DROPPED A LINER ON HIS RT. MIDDLE FINGER CAUSING A CONTUSION.
EE WAS ATTEMPTING TO KNOCK THE SAND BUILT UP ON THE SIDES OF THE SAND BIN WHEN HE RAN HIS HAND ACROSS THE TOP OF THE BIN AND CUT HIS HAND ON A STEEL BURR.
EMPLOYEE WAS ATTEMPTING TO PICK UP SOME SCRAP IRON AND STRAINED HIS BACK.
EMPLOYEE WAS LIFTING RAILROAD TIES TO BE USED FOR CRIBBING AND CARRYING THEM TO LOAD INTO TRUCK AND STRAINED HIS BACK.
