EE was exiting the office and reaching to close the front door when another employee saw the door ajar and went to close the door. When the other employee closed the door EE's left middle finger and left ring finger pinched in the door and caused fracture of distal phalanx and finger lacerations with complication.

Employee was changing a roller on a conveyor. He was havnig trouble getting a nut off of a bolt which held the roller. When the nut cam loose, the roller came loose and employee tried to hold it from falling.
