A 992 operator strained back as EE was sitting down in the operator's seat. EE was getting into the seat to adjust it and start the machine when EE felt discomfort in lower back. An onsite EMT was called to examine the employee. Upon further examination, a doctor would not release EE to full duty due to a back strain.

Employee was climbing into cab of truck, had his left hand grasping the door iron and right hand grasping the iron on other side as he was pulling himself up into cab he felt a twinge in his left arm bicep.

Nu-West Industries Inc · Caught in, under or between a moving and a stationary object

Employee was changing cutting edges on a D10 dozer with the assistance of two mechanics. The bar that the injured employee was using slipped and the cutting edge came down crushing the employees right ring finger in between the edge and the dozer blade, amputating the tip of the finger off.

Employee was installing one section of a rolled dozer liner, setting the liner into place and as he moved the plate towards the face of the dozer, the lifting eye that was welded on the top edge of the liner broke free hitting the dozer face, and sliding onto the welder's left foot. X-ray taken on 7/11/12 indicated a non-displaced fracture of a metatarsal bone in left foot.

EE WAS HELPING TO MOVE 4" IRRIGATION PIPE. WHEN THIS WAS COMPLETED HE COMPLAINED OF BEING VERY TIRED AND WEAK. HE CALLED IN SICK THE FOLLOWING TWO DAYS. ON 4/18 HE WAS ADMITTED TO THE HOSPITA L. HE WAS DIAGNOSED WITH KIDNEY FAILURE. HIS DR.CONCLUDED THAT THIS WAS DUE TO OVER-EXERTION OF HIS MUSCLES ON 4/15/02.
