WHILE MOVING A CRUSHER LINER, RIGGING SLIPPED & CRUSHED EMPLOYEE LEFT FIRST & 2ND TOES

Lead Technician was on their second day working alongside Field Technician to complete field assembly of an HD605-8. Work is being performed inside the equipment maintenance shop. Technician finished welding the left side tire guard on. Technician was using a cloth and solvent to wipe the welded area when they noticed something had entered left eye from behind safety glasses.

Metal backplate crushed left index finger

Stone chute was lying on conveyor #10A, tried to lift chute with one hand when EE felt a pop and then fingers tinging.

Employee was moving a heavy duty jack stand when EE lost EE's balance and fell to the concrete floor in the shop. The jack stand landed on EE's right hand causing a laceration that required 7 stitches to close.

Employee was removing a chain that slipped off the lower drive sprocket of a stacking conveyor motor. After locking out the drive motor, employee was removing chain on "slack" side of upper sprocket. As employee did this, the tension on the opposite side, pulled the chain and employee's hand and pinched it between the chain and the sprocket.

Field Service Technician performing work on a piece of heavy equipment. Just removed a strap from truck crane to hydraulic cylinder. When trying to remove strap completely from cylinder, the cylinder dropped and pinched right wrist.

Bearing on head pulley slipped pinching finger.

Pulling gate closed at the end of the day and felt pain in left shoulder, was originally reported as 1st aid. Employee is now treating with physical therapy as prescribed by physician making this re-portable.

Employee got his right middle finger pinched between screen panel and screen frame.

Cut finger on sharp bolt while changing cutting edge.

Employee bumped right knee on steel channel.

Replacing guard on drive shaft of loader, guard slipped pinching left ring finger between guard and truck frame.

EE was lifting the welding trailer off the hitch of a pickup truck and felt soreness in his lower back.

While unplugging a feeder in the surge tunnel a rock rolled down striking the EE in the left hand causing a laceration requiring 4 stitches.

While climbing loader ladder, EE felt a sting in the stomach area. As the day went on, the pain got worse. He went to the hospital and they said he had a stomach hernia. Referred to a surgeon.

Moving screen cloth into position, screen slipped out of hand and sliced thru work glove and into left thumb. Required four stitches in thumb.

Unplugging blocked stone chute. He was utilizing a steel hook to pull at the stone and felt a "twinge" or pull in the groin area.

EE was blowing off and cleaning 4" ball valve. Piece of metal blew in eye. Did not go for medical treatment till 12/13/07, because he believed it was sand in eye and he had washed it out on night of 12/10/07.

Miner was changing a belt on a crusher, that was locked and tagged. As he was changing the belt, the belt slipped and caught his finger between the belt and pulley. He had to have 4 stitches and broke the bone on his left pointer finger.

Employee was handling lifting straps, twisted left wrist resulting in the need for surgery at a later date to repair tendon/ligament damage.

EE was working on a portable plant. At the end of the day he told me that his back was sore, but he did not need to go to the doctor at that time. He went the next day. It was a back sprain.

EE was unloading a pallet of parts pallet started to shift employee put hand behind pallet to steady it pallet shifted pinching employee's right index finger.

Pinched finger moving rock.

Pinched thumb in clam gate.

Changing plate in chute, plate slipped struck EE on left ring finger.

While replacing screen cloth panel back in storage, panel shifted pinching ee's left pinky finger betwen handrail and panel cutting his finger.

WHILE CLEANING MATERIAL UNDER PRIMARY PLANT, EE HIT HIS HEAD ON A STEEL BEAM. HE WAS WEARING A HARD HAT AT THE TIME OF ACCIDENT.

EE WAS BENDING OVER AND SHOVELING DIRT - WHEN HE HEARD A POP SOUND FOLLOWED BY PAIN & STIFFNES IN LOWER BACK.

PLATE WAS BEING LOWERED INTO PLACE AND SLIPPED. IT FELL APPROX. 2 FEET. IT PINCHED FINGER BETWEEN PLATE AND ROCK. FINGER WAS BROKEN.

EMPLOYEE SPRAINED BACK MUSCLES WHILE REMOVING FOOTBALL SIZED STONES FROM SCREENS UNIT AT PORTABLE CRUSHER.

EE JAMMED STEEL TOE OF SHOE INTO RIGHT FOOT WHILE GETTING OFF LOADER (WHILE SERVICING LOADER). EE WAS CLIMBING DOWN OFF THE LOADER WITH FILTERS IN HIS HANDS AND COULD NOT MAINTAIN 3 POINTS OF CONTACT.

EMPLOYEE WAS HOSING OUT A CHUTE CLOGGED WITH MUD. A LARGE CLUMP OF MUD FELL HITTING HIS WRIST PINCHING IT IN THE OPENING OF THE CHUTE. INJURIES INCLUDED A BROKEN LEFT ARM AND LACERATION REQUIR ING STITCHES.
