Lead Technician was on their second day working alongside Field Technician to complete field assembly of an HD605-8. Work is being performed inside the equipment maintenance shop. Technician finished welding the left side tire guard on. Technician was using a cloth and solvent to wipe the welded area when they noticed something had entered left eye from behind safety glasses.

Employee was moving a heavy duty jack stand when EE lost EE's balance and fell to the concrete floor in the shop. The jack stand landed on EE's right hand causing a laceration that required 7 stitches to close.

Employee was removing a chain that slipped off the lower drive sprocket of a stacking conveyor motor. After locking out the drive motor, employee was removing chain on "slack" side of upper sprocket. As employee did this, the tension on the opposite side, pulled the chain and employee's hand and pinched it between the chain and the sprocket.

Field Service Technician performing work on a piece of heavy equipment. Just removed a strap from truck crane to hydraulic cylinder. When trying to remove strap completely from cylinder, the cylinder dropped and pinched right wrist.

While climbing loader ladder, EE felt a sting in the stomach area. As the day went on, the pain got worse. He went to the hospital and they said he had a stomach hernia. Referred to a surgeon.

EE was blowing off and cleaning 4" ball valve. Piece of metal blew in eye. Did not go for medical treatment till 12/13/07, because he believed it was sand in eye and he had washed it out on night of 12/10/07.

Miner was changing a belt on a crusher, that was locked and tagged. As he was changing the belt, the belt slipped and caught his finger between the belt and pulley. He had to have 4 stitches and broke the bone on his left pointer finger.
