Employee was moving 4'x8' screens on tower 1 with another employee. They lifted the screens about 2 inches off the ground and approximately 10ft down the catwalk. Shortly after this process was completed the employee felt a strain in EE's stomach and noticed a protrusion from EE's belly button. Umbilical hernia.

Showing all 11Employee was grabbing a 5-gallon bucket of oil off the spill containment in the lube shed and placing it into the back of the pit truck when their right middle finger got wedged between the bucket and the handle injuring their finger.
Employee was operating the dragline when the wedge came out of the dump cable socket. As the employee began to reinsert the cable back into the socket with the help of another employee, the short end of the cable popped out of the socket and hit EE in the chin. Received 4 stitches to the chin.
While checking the top deck of the grizzly, employee tried to move a stone that was lying on top of the grizzly bars. He heard and felt a tear in his left biceps. Employee was rechecked on 07/08/2013 and put on restrictive duty.
Employee was pulling himself out of a chute and felt discomfort in his left shoulder. Employee was rechecked on 3-28 on put on light duty.
While replacing a tail pulley on the lignite conveyor, employee tried to move a gear box out of the way and felt a pull/pain in his lower back. Went to Wixom Occupational Health Center on 5/12/06 where they distributed 2 bottles of pills to him. Naproxen (500 mg) 14 pills and Skelaxin (800 mg) 12 pills.
While wiring up a motor, employee picked up a piece of wire and his safety glasses dropped down on his nose and the wire then hit his left eye.
While lifting gate weights for placement onto edge of screen, employee felt a pull in his lower back.
WHILE WORKING ON A BEARING, A PIECE OF STEEL HIT THE EE'S FOREHEAD JUST ABOVE HIS SAFETY GLASSES. EE DIDN'T REALIZE THAT HE HAD ANYTHING UNDER THE SKIN UNTIL 7/25/03 AT WHICH TIME HE WENT TO THE CLINIC AND THEY REMOVED THE FOREIGN BODY. NO DAYS LOST AND NO RESTRICTED DUTY.
SCREEN PANEL FELL OUT OF ANOTHER EE'S HAND WHILE INJURED EE WAS PASSING IT UP TO HIM ON THE NEXT LEVEL AND IT CUT THE END OF HIS NOSE AND PUFFED UP HIS LIP. HIS NOSE REQUIRED SUTURES THAT ARE TO BE REMOVED IN 7 DAYS.
EE WAS INSTALLING A MUFFLER ON LOADER(ELI5). WHILE TWISTING IN A POSITION TO INSTALL MUFFLER, HEFELT A PULL IN HIS STOMACH AND THEN A SLIGHT PAIN.
