Employee was straightening a piece of pipe by hand. The pipe kicked back striking right thumb. Went to Dr. 6/12 and was released to full duty. Went back to Dr. on 6/20 and was released to work with restrictions and a brace with possible torn ligament in right thumb.

Employee was moving a pump hose when employee felt something pop in shoulder. Employee was seen by a doctor and returned to work and was able to perform normal duties. Employee worked normal job until 11/2/2020, when employee was scheduled for surgery on 11/3/2020 causing employee to miss work.
Employee mashed finger when lowering a bearing assembly to the press table. A block slipped from under the assembly mashing EE's finger between the assembly and the stand. Diagnosis was a fractured distal phalange (fingertip) no restrictions or treatment. EE was wearing leather work gloves at the time of the injury.
Employee was holding a small hydraulic pump (approx. 2 lbs) with arm extended for an extended time. EE complained of shoulder pain, went to doctor returned to work full duty. Worked until 2/7/19 when EE went to doctor. EE was placed out of work and referred for rotator cuff surgery from a strain.
Employee stated ee may have hurt shoulder pulling on a basket the shift before off days. The basket weighs approx. 23 pounds and is placed in the top of a tanker truck to catch oversized material. Employee was off 7 days for scheduled time off and did not report the incident until the day ee was scheduled to return.
Opening a metal can of welding rods. Cut hand on lid.
Employe got lime dust in left eye while measuring Kiln silos lowered tape measure into measuring port causing dust to blow up in face and under glasses.
Employee strained right elbow while installing water pump. Employee was pushing/pulling during installation.
Employee suffered a strain to shoulder while pulling a rubber hose water line.
Employee bent over to pick up a sample screen in the lab and felt a burning sensation in lower back.

Employee was attempting to unload a chute from a screen and lime fell onto his safety glasses. As he removed his safety glasses, lime got into his eye.
Employee was helping co worker remove screw flights from a screw trough in the sizing system weighing approximately 150 lbs. He felt a pain in his right lower back and hip.
Operator was walking through the bottom floor of the product silos to adjust feeder controls and dry lime got into his left boot due to holes from wear. Socks were wet due to walking in water earlier in the day. Employee did have a new pair of boots at home, but didn't wear them. Did not become reportable until 4/20/15
Employee was sizing a metal patch for a fan housing. His finger got sucked into the hole and cut the tip of his left, middle finger at the joint.
EE was Walking under the kiln bag house on his way to stone tunnel. His hard hat had fallen off and as he stood up while under the steps hit the top of his head on a sharp metal end of angle that's supports the steps. EE received two staples at the ER.
Employee was climbing up a permanent ladder in the conveyor area the employees shoulder began to hurt. Employee went to the doctor on 8/13/13.
EE was making repairs to the bend pulley. After unlocking the belt, and then verifying that the belt conveyor was running correctly, he gave instruction for to turn the belt off so he could put the guard back on. EE was picking up a hammer, which got caught by the belt and EE's right arm was caught between the shaft of the bend pulley and the conveyor truss.
On 7/24/2012 at about 11:45 pm an employee and injured employee were replacing idlers in the 5A belt. IE was pulling on the old idler to free it when it came loose all at once and fell and hit IE on his right foot.
Working on bucket elevator putting chain together pulled on tensioner and pulled muscle.
He was shoveling lime off plate, the wind blew lime dust in his eyes.
Employee was closing the steel double doors from inside the building located on the bottom floor of the hydrate building, when his right thumb became pinched between the two doors.
Employee was rewiring a water pump on a floating pontoon. When we tried to "whip" the slack out of the cable (large diameter), he felt a pull in his left elbow. Employee was referred to an orthopedic and discovered a tendon tear at the bicep. Surgery is being schedule and will cause lost time.
Employee was loading roof bolts into the carousel of the roof bolter. The carousel became hard to move, he jerked on it to make it move and felt a pain in his lower back.
EE was trying to unplug a hose. While doing this, he got lime in his eye.
Employee was turning the cap that holds the oil in the trunion bearing. The cap was stuck to the shut off valve and would not release. He continued to try and twist the cap off. When it gave way he reported feeling a strong burning pain in his groin.
As employee was bending over to hold a 5" x 24" piece of metal channel being cut felt a pull in lower left back.
Employee stooped down to pick up a box weighing approximatley 20 lbs. As he raised from stooped position, twisted slightly and felt a pain in his lower back.
Employee was lowering a spout to load a truck with Lime Kiln Dust. The spout needed to be adjusted to go into the truck hatch properly. The employee jerked the spout with the cable and felt a sharp pain in his right groin area. Doctor determined it to be a hernia.
Contract Truck driver was closing tailgate on truck when he slammed his left middle finger in between the truck and the tailgate. Driver ended up with 5-6 stitches in finger.
EE was carrying 4 foot tubes of resin into the roof bolter. One of the tubes got hung and EE twisted causing pain in his right side down his right leg. EE did not loose any time until 7/5/05. EE is going in for further testing and might lose time again.
EE was raking gators off the grate at the end of the kiln when he felt pain in his left arm, lower back and left leg. EE went to the doctor and was released to full duty till the root cause of the pain was determined. EE will be having orthoscopic surgery on April 26, 2005 on his left shoulder. This will be the first time he will have missed any work due to this injury.
The tailgate on the back of the Mack dump truck would not latch properly. In order to get tailgate to latch employee pulled the tailgate up with one hand and lifted the latch with the other. The tailgate slipped out of his hand and smashed the middle finger on his left hand. Employee required 6 stitches.
EE WAS CLIMBING DOWN A STATIONARY LADDER ON TOP OF PRODUCT SILOS. UPON REACHING THE BOTTOM HIS FOOT CAUGHT ON THE FLANGE OF A CHUTE LOCATED NEXT TO THE LADDER. HE CHIPPED A BONE.
EE WAS LOADING BOLTING RACK FROM THE FRONT BASKET ON THE BOLTER. THE TIE WRAP ON THE BOLTS BROKE AND HE PULLED UP CAUSING PAIN IN HIS BACK. IT WAS NOT REPORTED BEFORE NOW BECAUSE HE WAS ABLE T O DO HIS JOB UNTIL TODAY (7/30/02).
SHOVELING MUD FROM FLOOR TO BELT WHEN HIS ARM/BACK STARTED TO HURT.
ON 4-23- EE WAS PUTTING BELT CLAMPS ON A CONVEYOR. HIS SHOULDER POPPED CAUSING PAIN . ON 4-25 EE WAS PULLING ON A WRENCH WHICH MAD HIS NECK AND SHOULDER SORE. EE WAS ABLE TO CONTINUED DOING HI S JOB UNTIL 8-12 WHEN HE WAS PLACED ON RESTRICTED DUTY.
EMPLOYEE WAS WORKING WITH ANOTHER EMPLOYEE TO LIFT A SCREW COVER ABOVE HEAD TO LAY DOWN ON SCREW HOUSING. SAME SHOULDER AS WAS INJURED PREVIOUSLY. CLAIMS IT NEVER STOPPED HURTING.
EE WAS AT WORK WHEN HE FELT A CRICK IN HIS NECK. DID NOT REPORT UNTIL 4-14-02. WAS NOT DOING ANYTHING AT THE TIME HE FELT THE PAIN. HAD BEEN INSTALLING A HYDRAULIC HOSE ON A JUMBO DRILL TWO HO URS EARLIER.
EE WAS LIFTING A LID ON A TANKER TRUCK WHEN HE FELT A PULL IN HIS BACK. HE DID NOT REPORT THE INCIDENT AS WORK RELATED TO THE CO UNTIL APRIL 10 , 2002.
EE WAS STARTING A DIESEL PUMP. HE PULLED ON THE START ROPE AND HAD A CATCH IN HIS SHOULDER.
LOADING BOLTS ONTO THE ROOF BOLTER WHEN HE FELT A STING IN HIS BACK. IT STARTED HURTING LATER.
EE WAS DROPPING DOWN RAIL CARS. WHEN HE RELEASEDTHE HAND BRAKE IT JERKED HIM AROUND TO ONE SIDE,MADE HIM SLIP AND TWIST HIS BACK.
WHILE SHOVELING MUD AND STONE, EE HURT HIS BACK.
EE WAS LOWERING A POWER CABLE DOWN THRU A 4" PIPE. THE CABLE WAS ATTACHED TO CABLE GRIP THAT WASHELD BY A CRANE.THE CABLE GRIP CAME LOOSE & CABLE FELL STRIKING EE ON TOP OF HIS HARD HAT
EE WAS ON A TANKER GETTING READY TO LOAD IT WHENHE REACHED FOR THE CONTROL. DUST FELL OFF THE SPOUT AND INTO HIS EYE. THE EYE WAS FLUSHED OUT WITH EYE WASH. HE WAS THEN TAKEN TO THE HOSPITAL. HIS EYE WAS BURNED.
EE WAS CLEANING OUT FROM UNDER THE CONE CRUSHER DISCHARGE BELT. A ROCK COME OFF THE BELT OR PLATFORM ON TOP OF THE CONE CRUSHER. THE ROCK KNOCKED HIS HAT OFF AND CUT HIM. HE HAD A 1 1/2" LACER ATION THAT WAS TREATED BY THE DR AT ST. MARY'S E.R. IT TOOK FOUR STAPLES.
INDIVIDUAL WAS CLEANING UP BOXES AND PALLETS AND STEPPED ON A ROCK AND TURNED HIS ANKLE AND KNEE.
INDIVIDUAL WAS BENDING OVER WHILE INSTALLING A SENSOR IN TRANSMISSION OF A MINE TRUCK. WHEN HE STOOD UP, HE FELT PAIN IN HIS LOWER BACK.
