Employee was working on a piece of motor when his lower left leg hit a piece of metal on the motor causing a laceration to his leg. The employee self treated the injury and did not report the injury until 07-28-08. The doctor's report was delayed in the mail system and was not found by the ALPM until 10-24-08.

Employee was climbing ladder to operator's cab of a Cat 60 ton haul truck when he felt a pain in his lower back.

Employee exited tri-axel dump truck at customer site, turned to close door of truck, bumped elbow on edge of door.

Employee was attempting to loosen a conveyor idler when his left hand slipped, pinching his finger between conveyor & idler.

EMPLOYEE WAS LIFTING & CARRYING A 48" X 52" SCREEN PANEL (APPROX.50 LBS.) WHEN HE FELT A PULL IN HIS GROIN, RESULTING IN A HERNIA. EE REPORTED INJURY THE DAY OF OCCURANCE, CONTINUED TO WORK LIGHT DUTY, OPERATION SCHEDULED 6/1/05.

EMPLOYEE WAS ASSISTING WITH A CRUSHER BAR REPLACEMENT. WHEN HE REMOVED THE ANTIROTATING DEVICE TO REPOSITION THE BAR HIS RIGHT FOOT WAS PINCHED BETWEEN THE BAR & HOUSING.

EE WAS CLEANING OUT CONVEYOR JAM WHEN HIS LEFT FOREARM CAME IN CONTACT WITH THE STRUCTURE.

EE WAS LOADING A BULK TRUCK WHEN HE BUMPED HIS RIGHT SHOULDER ON A STEEL BEAM.
