Driver was having trouble with the automatic tarp on their trailer. Driver climbed onto the step on the trailer and started working on the wiring going to the motor that controls the tarp. The driver fell from the step and landed on their back ion the ground of compacted crushed limestone.

Two Maintenance Technicians were unloading 8 X 8 X 48 long Treated lumber wood blocks for the track pedestrian crossing. They were sliding blocks out of the back of a pickup, when one of the blocks slipped out of the grip of the injured EE. The block fell onto their right thumb. The injured EE went to the ER and diagnosed with a hairline fracture of the first digit of the thumb.

The injured employee was helping a another Miner smash empty 55-gallon drums, and putting them in the loader bucket. EE lifted an empty drum which weighed approximately 20 lbs, and twisted body to put it in the bucket. EE then felt a sharp pain go up EE's back.

Employee was performing an alignment on a lime kiln when they came in contact with Lime dust which resulted in lime burn to right arm and neck. Employee reported this incident on 4/1/20.

Injured employee and another mechanic were moving a coupling shaft to the back of a flatbed truck when the injured employee's right pinkie finger was caught by the grid coupling which cut through the glove and finger. Total weight of the coupling was about 100-110 pounds. The coupling teeth cut through the glove and into the right pinkie finger.

Injury occurred 09/17/18, but did not become reportable until 11/30/18 (dental care reqd). EE (injured) and another mechanic completed welding in the screenhouse. Co-worker accidentally dropped a bag of welding rods onto EE's face, causing a broken upper tooth. Dental care in late Nov was reqd.

A Miner was lifting a 12" dia X 20 ft long piece of steel pipe from a dumpster on the 13th level of the mine. The pipe hung on the edge of the dumpster and broke loose, striking the injured employee on the left shoulder. Injured employee was standing too close to the lift zone.

Employee completed loading lime into a railcar. He was in the process of closing the doors on the top of the railcar. The door hung and he overexerted his back as he attempted to close the door. Injury occurred Aug 30. Employee waited to visit the Dr. on Sep 1. No Dr.'s report was provided to EHS Manager until Sept 18th. Injury reclassified as MSHA reportable Sept 18th.

Injured employee was checking the Kiln #3 fire box for scale. As he was closing the steel door, he caught his left pinky finger between the door handle and the latch.

Employee was helping other employees unload 150 each 50 lbs bags of Anfo explosives on the 2nd level of the mine. Employee overexerted his left shoulder and was diagnosed with a strain of the left shoulder and was given restricted duty.

EE was drilling a starter hole in the duct work. The drill left a piece of metal shaving. EE had gloves on, but when he tried to pull the duct work, a piece of the metal cut his finger.

EE was working at the plant on 1-6-2011. The EE did not complain of any lime in his eye while he was at work. EE left work at his scheduled quitting time, eye seemed irritated, called the plant and was sent to see the eye doctor, where a piece of lime was removed. EE returned to work.

Employee pulled on a concrete form to strip it from the concrete. He did not report that he was in pain at the time. He worked his shift and returned to work the next day and worked. He was in obvious discomfort and was required to seek medical evaluation. The doctor said that he had a strained or damaged rotator cuff injury and laid the employee off for 2 weeks.

Working on the coal crusher and a sledge hammer lying on an elevated surface fell, contacting his left hand. Went emergency room for x-rays but nothing was broken. Doctor placed him light duty for 10 days.

Moving gantry crane to support loader bucket and employee states he felt his knee pop. This is the same knee he had surgury on in 2008. It was thought this was an aggrevation of an existing condition. Went to the doctor on February 5, 2009 and doctor said it was a new incident.

A metal plate was cut with a torch leaving jagged edges. The employee was attempting to help move the metal when his wrist came into contact with one of the jagged edges cutting it.
