EMPLOYEE WAS PUSHING AN OPEN HOPPER TO HELP IT DUMP WHEN HE OVEREXERTED HIMSELF AND AGGREVATED AN OLD INJURY (HERNIA). ORIGINAL INJURY OCCURRED APRIL 12, 2001 WHILE EMPLOYED AT IMV. PART OF BODY AFFECTED WAS THE RIGHT GROIN AREA. ASSOCIATE WORKED THE NEXT SCHEDULED SHIFTS THEN CALLED IN SORE MONDAY MORNING AND MISSED THE NEXT TWO SCHEDULED SHIFTS.

AT ABOUT 7:30 AM EE CAME BY THE WINDOW TO MY OFFICE & KNOCKED. I WENT OUTSIDE TO SEE WHAT WAS NEEDED. EE STATED THAT HE HAD PINCHED HIS SCIATIC NERVE & THAT HE COULD NOT CLIMB THE STEPS. ASKED EE SPECIFICALLY OF OR WHAT HE DID TO HURT HIS BACK. HE REPLIED: NOTHING HE COULD DIRECTLY PLACE HIS FINGER ON. HE WAS GOING TO HIS CHIROPRACTOR.
RIGHT KNEE SWOLLEN, SORE AND STIFF AFTER CHANGING SCREENS ON SCREEN DECK, PUSHING, PULLING AND AT TIMES KNEELING ON SCREENS & FRAME REQUIRED. INCIDENT WAS REPORTED TO SUPERVISOR THE FOLLOWING WEEK. RECEIVED INCIDENT REPORT ON 9/10/02, RECEIVED DR.'S REPORT WITH RESTRICTIONS ON 9/26/02(LAST SCHEDULED SHIFT). NO REASONABLE ACCOMODATIONS CAN BE MADE, EE OFF DUTY TILL FULL RELEASE STAR
