On 11/20/2025 at Melvin Stone, employees were removing the bucket from EX1200 (22030002). When the final pin was pressed out, the stick settled and caught EE's left hand between the stick and bucket cylinder. First aid was given and EMS transported EE for treatment. No fractures reported.

Changing wear bars in head box. When the bolts were loosened the bar flipped hitting the thigh of the operator. The employee was given an Ibuprofen injection by the attending physician at the ER.
Employee was operating a propane fueled forklift. The engine stopped. The employee thought the propane tank valve was closed. While attempting to unscrew the supply line, EE's left hand and fingers were burned by the cold propane release.
Employee had locked and tagged out the crusher to adjust the curtain. The rotor was on the "up-stroke" and the employee pushed the rotor over with their hand rather than using a board or steel bar and EE's glove got caught between the rotor and housing. This caused a cut to EE's right thumb requiring 6 stitches.

WHILE DOING SO, A ROCK ABOUT 75-100LB CAME LOOSE FROM THE ROCKS PILED IN THE FEEDER. IT SLID DOWN THE PILE AND CAUGHT ONE EMPLOYEE'S LEFT THUMB BETWEEN IT AND THE STEEL FEED THROAT OF THE CRUSHER, BREAKING HIS LEFT THUMB.
REBUILDING SPOKANE CRUSHER, LINING UP THE TOP SHOE COVER PLATE. IT SLIPPED AND SMASHED HIS 3RD AND 4TH RINGERS ON HIS RIGHT HAND.
